손목밴드를 이용한 내관지압이 회복실 체류 중인 여성 갑상선 절제술 환자의 오심과 구토에 미치는 효과

Abstract

This study was carried out to examine effects of Nei-Guan(P6) acupressure wristband on nausea, vomiting, retching for female thyroidectomy patients. Using a non-equivalent control group posttest quasi-experimental design, 60 patients were assigned into three groups: the experimental group (n=20), received acupressure wristband on the point of P6 of one hand, the placebo group(n=20), received acupressure wristband on the simply on the wrist crease on one hand, and the control group(n=20), received usual care. Data were collected using Index of Nausea, Vomiting, and Retching(INVR) at three times: at the recovery room, at the 6 hours after operation, and at the 24 hours after operation. Results were as follows: The total INVR scores were significantly different among three groups overtime during 24 hours (F=2.50, p=.046). Scores of postoperative nausea were statistically significantly lower than those of the placebo or the control groups at the recovery room(F=2.58, p=.004). However, there were no differences in scores of postoperative vomiting or retching among three groups overtime. In conclusion, Nei-Guan (P6) acupressure wristband was found to be effective measure on nausea for female thyroidectomy patients.

본 연구는 손목밴드를 이용한 내관지압이 회복실 체류 중인 갑상선절제술을 받은 여성 환자의 오심과 구토에 미치는 효과를 검증하기 위함이었다. 갑상선 절제술이 예정된 20세에서 65세 사이의 여성 환자 60명을 대상으로 하였다. 대상 환자들은 마취유도 30분 전에 한쪽 내관 부위에 손목밴드를 거치한 실험군(n=20), 한쪽 손목 금 부위에 손목밴드를 거치한 위약군(n=20), 일반적인 간호처치만을 제공한 대조군(n=20)으로 하였다. 실험처치의 확산을 방지하기 위하여, 대조군, 위약군, 실험군 순으로 시기를 달리 하였으며 오심구토헛구역질 측정은 회복실 퇴실 시, 수술 후 6시간, 수술 후 24시간의 세 차례에 걸쳐 측정하였다. 연구도구는 Rhodes와 McDaniel이 개발하고 김영재 등(2000)이 번역한 오심구토헛구역질 측정도구(Index of Nausea, Vomiting, and Retching)를 사용하였으며, 자료분석은 SPSS/PC(Version 12.0) Program을 이용하여, 기술통계와 repeated measures of ANOVA로 분석하였다. 시간 경과에 따른 세군의 오심구토헛구역질 점수는I통계적으로 유의한 차이가 있었다(F=2.50, p=.046). 시간 경과에 따른 세군의 오심 점수는 통계적으로 유의한 차이가 있었으나(F=2.58, p=.004), 구토나 헛구역질은 시간경과에 따라 세 군간에 유의한 차이가 없었다. 결론적으로 손목밴드를 이용한 내관지압은 갑상선 절제술을 받은 여성 환자의 회복실 체류 동안 오심을 완화하는 효과가 있는 것으로 나타났다.
