간호사의 암성 통증관리 지식 및 수행 실태 : 암성 통증관리지침 권고안을 중심으로

Abstract

This research is a descriptive study to understand the knowledge and perfomance of cancer pain management in clinical nurses based on Cancer Pain Management Guideline(2008) developed by Korean Ministry for Health, Welfare and Family Affairs. The subjects were 364 nurses from medical or surgical nursing units of three tertiary hospitals and six secondary hospitals in Daegu, Kyungbuk area. Two instruments were used to measure the knowledge and performance, which were developed based on Cancer Pain Management Guideline. Data were collected from september 16 to october 7 in 2009. Data were analyzed using SPSS WIN 17.0 program. Important findings of this study were as follows. 1. Total mean score in the knowledge of cancer pain management was 74.50. Total mean score in the perfomance of cancer pain management is 3.03. 2. There were significant differences in the knowledge and perfomance of cancer pain management according to the subjects' age, total length of service, field of service, position, educational background, religion, state of marriage, experience of receiving education about pain management, the number of receiving education about pain management, length of service for cancer patients, recognition of Cancer Pain Management Guideline and so on. 3. There were significant relationship between the knowledge and perfomance of cancer pain management(r=.324, p<.001). The field of service, recognition of Cancer Pain Management Guideline, state of marriage, number of receiving education about pain management, explained 29.1% of perfomance of cancer pain management. In conclusion, it is showed that they conduct the perfomance of cancer pain management more as they have higher the knowledge of cancer pain management. It is found that it has the more influence on knowledge and perfomance of cancer pain management as they notice the more Cancer Pain Management Guideline and the more educated they are about pain management. Accordingly, it is considered that it is required continual educations based on Cancer Pain Management Guideline in order to conduct organized and effective pain management. It is considered that the result of this study would be basic materials for developing a program of cancer pain management as well as contribute to nursing practice, research and development.

본 연구는 보건복지가족부의 암성 통증관리지침 권고안(2008)을 중심으로 암성 통증관리에 대한 간호사의 지식 및 수행 실태를 파악하기 위한 서술적 조사연구이다. 연구대상은 대구광역시 소재 3개 종합전문요양기관, 3개 종합병원과 경상북도 소재 3개 종합병원에서 근무하는 간호사 364명을 대상으로 실시하였다. 본 연구에서는 암성 통증관리 지식 및 수행 실태를 조사하기 위해 암성 통증관리지침 권고안(2008)을 중심으로 본 연구자가 개발한 조사도구를 사용하였다. 자료 수집 기간은 2009년 9월 16일부터 2009년 10월 7일까지 실시하였다. 수집된 자료는 SPSS WIN 17.0 프로그램을 이용하여 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, Duncan test, Pearson Correlation, Stepwise multiple regression으로 자료 분석하였다. 1. 전체 암성 통증관리 지식의 총점은 74.50점으로 나타났다. 영역별로 비약물요법에 대한 지식 86.95점으로 가장 높았으며, 암성 통증에 대한 지식 75.11점, 약물요법에 대한 지식 71.98점의 순으로 나타났다. 2. 전체 암성 통증관리 수행 실태의 평균평점은 3.03점으로 ‘대체로 한다’ 정도에 해당하는 수행 실태가 나타났다. 영역별로 통증평가 수행 3.12점, 통증중재 수행 3.08점, 통증사정 수행 2.91점의 순으로 나타났다. 3. 대상자의 연령, 총 근무연수, 근무분야, 직위, 최종학력, 종교, 결혼상태, 통증관리 교육받은 경험, 통증관리 교육받은 횟수, 암환자 간호 근무연수, 암성 통증관리지침 권고안 인지 등이 지식과 수행 실태에 유의한 차이를 나타냈다. 4. 대상자의 암성 통증관리 지식과 수행 실태(r=.324, p<.001)는 정적 상관관계가 있는 것으로 나타났다. 암성 통증관리 수행 실태에 대해 근무분야, 암성 통증관리지침 권고안 인지, 결혼상태, 통증관리 교육받은 횟수, 종교가 통계적으로 유의한 29.1%의 설명력을 나타냈다. 결론적으로 암성 통증관리 지식이 높을수록 수행 실태가 높은 것으로 나타났다. 암성 통증관리지침 권고안을 인지하고 있을수록, 통증관리 교육을 많이 받았을수록 암성 통증관리 지식과 수행 실태에 영향을 주는 것으로 나타났다. 따라서 보다 체계적이고 효과적인 통증관리를 하기 위해서는 암성 통증관리지침에 기반한 계속적인 교육이 중요하다고 생각된다. 본 연구결과는 암성 통증관리 프로그램 개발 시에 기초 자료가 될 수 있을 뿐 아니라 간호실무 및 연구발전에 기여할 수 있을 것으로 사료된다.
