보건소 방문간호사의 노인 약물교육 수행도와 중요도 인식

Abstract

This study investigated the performance and importance awareness of medication education for the elderly by visiting nurses employed at public health centersy. The research subjects were visiting nurses working in public health centers located in Daegu, Gyungbuk, and Gyungnam regions. Data collection was done by distributing 215 questionnaires via mail from March, 29 to April 18 in 2010. For the 173 replied data, SPSS WIN 15.0 was used, for general and work-related characteristics of visiting nurse and drug education related characteristics of the elderly, frequency, percentage, average and standard deviation were used, and the relationship between performance and importance was analyzed with t-test. The results of this study were follow as: 1) Forty two respondents (24.2%) answered that they had been trained for medication education for the elderly while working as visiting nurses, which was relatively small numbers. Most of the visiting nurses were carrying out medication education which was individually done in less than 15 minutes. 2) It was investigated that visiting nurses were educating on drugs from time to time, and the performance of medication education was 2.19 point. The most frequently educated contents were about medications the elderly were taking, and medication time, purpose of medication, cautions in medication. 3) The importance of visiting nurse's medication education was 3.15 point, which appeared relatively higher than the performance of drug education. The educational contents in which the importance of drug education was high were purpose of medication, medication time, side effects of medication, and cautions in taking medicine. 4) There was no statistically significant difference in the performance and importance of medication education of visiting nurses according to general and work-related characteristics. 5) There was statistically significant difference in the performance and importance of visiting nurses according to medication education-related characteristics. Since the importance of drug education is higher than its performance from the results above, standardized educational materials and programs suitable for the elders at home should be developed in order to increase the performance, and policies that can strengthen visiting nurses' abilities on medication education for the elderly should be made.

본 연구는 보건소에 근무하는 방문간호사의 방문간호 노인 대상자에게 제공하는 약물교육 수행도와 중요도 인식을 조사하여 노인의 올바른 약물복용을 촉진 시킬 수 있는 중재법 마련을 위한 기초 자료를 제공하고자 실시된 조사연구이다. 연구대상자는 대구, 경북, 경남 지역에 위치한 보건소에 근무하는 방문간호사로 하였다. 자료수집은 2010년 3월 29일부터 4월 18일까지 우편을 사용하여 215부를 배부하였으며 회신된 173부의 자료를 SPSS WIN 15.0 프로그램을 이용하여 방문간호사의 일반적 및 업무 관련특성과 노인 약물교육 관련특성은 빈도와 백분율, 평균과 표준편차를 이용하였고 수행도와 중요도의 관계는 independent t-test로 분석하였다. 본 연구의 결과는 다음과 같다. 1) 방문간호사로 근무하면서 노인 약물에 대한 교육을 받은 경험이 있는 방문간호사는 응답자중 42명(24.2%)이 받은 경험이 있다고 대답하여 비교적 적은 수를 나타내었고 방문간호사 대부분이 약물교육을 실시하고 있으며 개별적으로 15분미만의 시간으로 구두설명을 통해 약물교육을 실시하고 있었다. 2) 방문간호사의 약물교육 수행도는 2.19점으로 가끔 약물에 대해 교육한다고 조사되었으며 노인이 복용하고 있는 약에 대한 교육을 많이 수행하고 있었다. ‘약물의 투여시간’, ‘약물의 효능’, ‘복용 시 주의사항’은 높은 약물교육 수행도를 나타냈다. 3) 방문간호사의 약물교육 중요도는 3.15점으로 약물교육 수행도보다 비교적 높게 나타났다. 약물교육의 중요도에서도 노인이 복용하고 있는 약에 대해 더 중요하게 인식하고 있었다. 약물교육 중요도가 가장 높았던 교육내용은 ‘약물의 효능’, ‘약물의 투여시간’, ‘약물의 부작용’, ‘복용 시 주의사항’ 순으로 높은 중요도를 보이고 있었다. 4) 방문간호사의 약물교육 수행도와 중요도에는 수행도(51.13점)가 중요도(72.21점)보다 높은 점수를 보였으며 수행도가 높은 항목과 중요도가 높은 항목에는 차이를 보였으나 통계적으로 유의하였다. 5) 방문간호사의 약물교육 관련특성에 대한 수행도와 중요도에서는 약물교육 시간 항목에서 통계적으로 유의한 차이가 있었다. 이상의 결과를 통해 약물교육의 수행도보다 중요도가 높으므로 수행도를 높이기 위해 재가노인에게 적합한 표준화된 교육자료와 교육프로그램이 개발되어야 할 것이며, 방문간호사들의 노인 약물교육 능력을 강화 시킬 수 있는 방안이 마련되어야할 것이다.
