입원 노인에 대한 간호사의 지식과 태도 및 간호 실천 : 보훈병원을 중심으로

Abstract

The purpose of this study was to investigate the nurses knowledge attitudes and practice toward the elderly patient in the veterans hospitals. The data collected was from March 3 to March 23, 2008. The research design was descriptive study. Veterans hospitals in .Seoul, Gwangju, Daejon, Busan and Daegu. The subjects consisted of 420 nurses date were collected by self-reported questionnaires. The data was analyzed by frequency, percentage, mean, SD, t-test, ANOVA, Scheffe test, and Pearson`s correlation coefficient using SPSS Win 14.0. The results of this study were as follows; 1. The average score of knowledge for elderly patient was 12.56 of 25 total, and this result shows relatively low. 2. the average score of attitude to old people was 3.22 of 7 total, and this result shows negative. 3. The average score of nursing practice was 3.22 of 4 total, and this result shows positive. 4. There were significant differences in the degree of the knowledge according to marital status (t=1.967, p= .049). 5. There were significant differences in the degree of the attitude according to age (F= 8.910, p = .000), marital status (t= 1.967, p= .049), and gross clinical careers(F =9.247, p= .000). 6. There is the significant difference in the degree of nursing practice according to nursing careers(t =3.323, p= .001) toward old people. 7. There is the significant difference statistically according to the psychological knowledge among the knowledge and attitude(r = .154, p= .002) toward elderly patient(r = .154, p= .002). Thus, the higher psychological knowledge was in the knowledge toward old adult, the more positive the average attitude was in range. Considering the above result, the level of veterans hospital nurses` knowledge toward old adult was relatively low. Although the average attitude score was in negative range, the average score of nursing practice was in positive range. The higher psychological knowledge toward elderly, demonstrated more positive attitude toward elderly. Therefore there is the need to develop the guidebook for aging health nursing and the ordered direct educational program for old adult in the hospital system to increase the knowledge and positive attitudes.

본 연구는 보훈병원에 근무하는 간호사의 노인에 대한 지식과 태도 및 간호 실천 정도를 파악하기 위하여 실시하였다. 자료수집은 2008년 3월 3일부터 3월 23일까지 이루어졌으며, 연구설계는 서술적 조사연구이다. 연구대상은 서울, 대전, 광주, 부산, 대구에 소재한 보훈병원에 근무하는 간호사 420명을 구조화된 설문지를 사용하여 조사하였다. 자료분석은 SPSS 14.0 프로그램을 이용하여 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, 사후검정으로 Scheffe test, Pearson's Correlation을 이용하여 분석하였다. 본 연구결과를 요약하면 다음과 같다. 1. 노인에 대한 지식 정도는 총25점 만점에 12.56점으로 지식정도가 낮게 나타났다. 2. 노인에 대한 태도는 7점 만점에 3.22점으로 부정적인 태도를 보이는 것으로 나타났다. 3. 노인 간호 실천정도는 4점 만점에 3.27점으로 긍정적 실천을 보였다. 4. 결혼상태(t=1.967, p=.049)에 따라 노인에 대한 지식 정도는 유의한 차이가 있는 것으로 나타났다. 5. 연령(F=8.910, p=.000), 결혼상태(t=-2.219, p=.027), 총 임상경력(F=9.247, p=.000)에 따라 노인에 대한 태도 정도는, 유의한 차이가 있는 것으로 나타났다. 6. 노인 환자 간호 경험(t=3.323, p=.001)에 따라 노인 간호 실천 정도는 유의한 차이가 있는 것으로 나타났다. 7. 노인에 대한 지식 중 심리적 영역과 노인에 대한 태도(r=.154, p=.002)간에는 통계적으로 유의한 상관관계를 보여 심리적 영역 지식이 높을수록 노인에 대한 태도가 긍정적인 것으로 나타났다. 이상의 연구결과를 통하여 보훈병원 간호사들의 노인에 대한 지식은 낮은 수준이고, 노인에 대한 태도는 부정적인 것으로 나타났지만 간호 실천은 긍정적인 것으로 나타났다. 노인에 대한 지식 중 심리적 영역 지식이 높을수록 노인에 대한 태도가 긍정적인 것으로 나타났다. 따라서 노인에 대한 지식을 향상시키고 태도를 긍정적으로 변화 유지시키기 위해서 노인 간호 실무 지침서의 개발과 노인에 대한 병원시스템내의 맞춤형 직무교육 프로그램 개발이 필요하다.
