Prevalence of Lung Cancer in Patients with Interstitial Lung Disease

Abstract

간질성 폐질환, 특발성 폐섬유화증과 관련된 폐암의 역학은 아직 정립이 잘 되어있지 않으나 간질성 폐질환은 섬유화를 동반하는 병변이 특징이며, 잠재적으로 반흔성 폐암으로 발전 가능하다. 이 연구에서는 결체조직질환 제외 간질성 폐질환, 특발성 폐 섬유화증, 결체조직질환 연관 간질성 폐질환, 그리고 만성 폐쇄성 폐질환에서의 폐암 유병률을 비교하였다. 연구는 건강보험 심사평가원 HIRA 데이터로부터 수집한 후향적 코호트 이며 2011년 1월부터 2011년 12월까지의 전체인구 중 외래환자 대상으로 층화 무작위 추출법을 이용한 표본 자료를 이용하였다. 총 1,375,842명 표본 자료 중 40세 이상으로 제한하여 670,258명을 대상으로 폐암의 유병률을 조사하였다. 간질성 폐질환에서 폐암의 유병률을 다른 질환과 비교하기 위해 폐암의 유병률이 높은 것으로 알려진 만성폐쇄성 폐질환을 대조군으로 선정하였다. 표본자료에서 ICD-10 진단코드를 이용하여 간질성 폐질환, 특발성 폐섬유화증, 결체조직질환 그리고 만성폐쇄성폐질환 환자들을 분류하였다. 연구집단 전체, 결체조직 제외 간질성 폐질환, 특발성 폐섬유화증, 결체조직 연관 간질성 폐질환 그리고 만성폐쇄성 폐질환에서 폐암 유병률은 연구인구 100,000명 당 각각 420, 7334, 7404, 7272 그리고 4721로 확인되었다. 결체조직 제외 간질성 폐질환에서 폐암 유병률은 만성폐쇄성 폐질환 환자와 비교하였을 때 유의하게 높았다. 앞으로 만성폐쇄성 폐질환 뿐만 아니라 간질성 폐질환 환자에서의 폐암 발생에 대한 더 많은 관심이 필요할 것이다.

Lung cancer epidemiology related to interstitial lung disease (ILD) with or without connective tissue disease (CTD) and idiopathic pulmonary fibrosis (IPF) is not yet established. ILD potentially develops into scar lung cancer. We aimed to explore lung cancer prevalence in ILD patients with or without CTD and IPF in comparison with chronic obstructive pulmonary disorder (COPD). We evaluated lung cancer prevalence associated with ILD and IPF using Korean Health Insurance Review and Assessment Service (HIRA) data from January to December 2011. This database (HIRA-NPS-2011-0001) was created using random sampling of outpatients; 1,375,842 sample cases were collected, and 670,258 (age ≥ 40) were evaluated. Patients were identified using the International Classification of Disease-10 diagnostic codes. Lung cancer prevalence rates per 100,000 person for the study population were 420, and those with ILD, IPF, CTD-ILD, and COPD were 7334, 7404, 7272, and 4721, respectively. Lung cancer prevalence was significantly higher in those with ILD than in those with COPD. More attention must be paid to lung cancer development in those with ILD as well as COPD.
