하지불안증후군 환자에서 약물치료로 인한 뇌의 기능적 연결성 변화

Abstract

본 연구에서는 치료 받은 적이 없는 하지불안증후군 환자에서 기능적 뇌 자기공명영상을 이용한 안정 상태의 기능적 연결성을 관찰하고, 이러한 하지불안증후군 환자의 시상 네트워크의 특징이 도파민 작용제로 치료 받으면 어떻게 변화되는지를 관찰하였다. 본 연구는 일차성 하지불안증후군 환자 32 명(치료 받은 적이 없는 하지불안증후군 환자 16 명, 도파민 작용제를 이용하여 치료를 받고 있는 하지불안증후군 환자 16 명)과 정상대조군 16 명을 대상으로 진행하였다. 기능적 뇌 자기공명영상의 촬영은 하지불안증후군 증상이 없는 낮 시간에 진행되었다. 분석은 Analysis of Functional Neuroimage software를 이용하였으며, 후외측복측핵을 seed로 하여 seed based method로 분석하였다. 치료 받은 적이 없는 하지불안증후군과 정상대조군을 비교한 결과와 비교해, 치료 중인 하지불안증후군과 정상대조군을 비교한 결과에서 차이가 없어진 영역은 약물이 병태생리에 영향을 준 결과이며, 치료 중인 하지불안증후군과 정상대조군을 비교한 결과에서 추가적으로 차이를 보인 영역은 도파민 회로로서 하지불안증후군 증상 호전에 영향을 주는 영역이라 해석된다. 특히, 치료 받은 적이 없는 하지불안증후군과 정상대조군의 비교에서 차이를 보인 영역 중 치료 중인 하지불안증후군의 연결성 강도 값이 정상대조군의 연결성 강도 값에 가까워지는 패턴을 보이는 것은 하지불안증후군의 병리학적 회로 중 약물에 의한 증상완화와 연관되어있다고 할 수 있다. 결론적으로 도파민 작용제는 병리학적 회로를 조정하고, 직접적으로 증상을 제어하며, 추가적인 회로를 활성화시킨다고 할 수 있다.

The aim of the study was to examine drug related alterations of the resting-state connectivity in restless legs syndrome (RLS) patients. Resting state functional magnetic resonance images were obtained from 32 idiopathic RLS patients (16 drug naïve and 16 on dopamine agonists) and 16 controls, in the morning when RLS symptoms were absent. The resting state connectivities were measured with the seed based method using Analysis of Functional Neuroimage software. The bilateral ventral posterolateral nuclei were the seed. Compared with the controls, the drug naïve RLS group showed thalamic connectivity to be reduced or enhanced within some areas. These altered areas in the treated RLS group were either reduced or enhanced, becoming more like the patterns of the controls’ connectivities. These results were derived from a drug related alteration of the resting state brain connectivity in RLS. These alterations were more related to the drug itself activated by the dopamine circuit, and there was also a tendency of change toward the patterns of the control group.
