전위인식역치를 이용한 원발성 하지불안증후군 환자의 정량적 감각검사

Abstract

Restless legs syndrome/Willis-Ekbom disease (RLS/WED) is a sensorimotor neurological disorder, especially aggravated at night. The purpose of this study was to investigate the sensory dysfunction in primary RLS/WED using the current perception threshold (CPT) test, compared to control group, also assessed the differences between the diurnal variations of CPT of RLS/WED patients. Thirty primary RLS/WED subjects and 30 healthy controls were enrolled in this study. Secondary RLS/WED was excluded through face to face interviews and nerve conduction study of all subjects. Neurometerⓒ for the CPT test was used to assess patients using three stimulation protocols, 2000 Hz, 250 Hz, and 5 Hz by stimulating both big toes. The CPT test was performed twice: Once during the daytime period and again in the evening. RLS/WED patients had lower mean CPT measurements for all three stimulation protocols in the evening than daytime, while the control group did not show any difference. RLS patients showed a lower current perception threshold in the evening than daytime. This finding suggests hyperalgesia to sensory perception with nocturnal predominance. The diurnal variation of hyperalgesia in RLS/WED patients indicates a central sensory processing disturbance rather than a peripheral disturbance.

하지불안증후군은 저녁 시간에 특히 악화되는 감각운동성 신경학적 질환이다. 이 연구의 목적은 전위인식역치 검사를 이용하여 대조군에 비하여 원발성 하지불안증후군 환자에서의 감각기능 이상을 조사하는 것에 있으며, 더불어 하지불안증후군 환자의 일중 변동을 전위인식역치의 밤낮 차이로 평가하였다. 30 명의 원발성 하지불안증후군 환자와 30 명의 성별, 연령 맞춰진 대조군을 대상으로 하였다. 면담 및 검사실 검사를 통하여 이차성 하지불안증후군 환자가 배제되었고 모든 대상에서 말초신경병증을 제외하기 위해 신경전도검사가 시행되었다. 대상의 양쪽 엄지발가락에서 전위인식역치 값을 2000 Hz, 250 Hz, 5 Hz 에서 각각 평가하였다. 이러한 과정이 대상 모두에게 오전에 한 번, 저녁에 한 번 각각 두 번 씩 이루어졌다. 낮 시간에는 환자군과 대조군 모두에서 전위인식역치에 차이를 보이지 않았으나 저녁 시간에는 환자군에서 대조군에 비해 더 낮은 전위인식역치를 보이는 경향을 보였다. 집단 내 밤낮 비교에서 대조군에서는 밤낮 차이를 보이지 않은 반면 환자군에서는 모든 전위에서 저녁 시간에 통계적으로 유의하게 더 낮은 역치 값을 보였다. 원발성 하지불안증후군 환자들은 낮에 비해서 밤에 더 낮아지는 전위인식역치 값을 보였다. 이것은 밤에 심해지는 통각과민을 의미하며 원발성 하지불안증후군 환자에서 통각과민의 일중 변동은 말초 신경장애보다는 중추성 감각신경 조절에 문제가 있음을 시사하는 것으로 생각된다. 이러한 결과는 원발성 하지불안증후군 환자에서 중추성 통증조절 기전에 이상이 있음을 뒷받침하는 근거가 될 수 있을 것으로 판단된다.
