안면골 골절 환자에서 Neurometer?를 이용한 술 후 삼차신경 회복 기간의 측정

Abstract

안면골 골절(관골 골절, 안와골 골절)로 인한 삼차 신경의 손상은 해당 환자에게 일상생활에 상당한 불편을 초래한다. 이러한 환자들의 치료 후 감각 회복 기간에 대해서는 충분한 연구가 이루어지지 않아 이에 저자들은 안면골 골절 환자를 대상으로 Neurometer® CPT를 이용한 후향적인 조사를 통해, 삼차 신경의 손상 후 회복 시기를 객관적으로 연구하고자 한다. 본원에서 2013년 12월부터 2015년 3월까지 관골 골절, 안와골 골절로 인해 수술(관혈적 정복술)을 시행한 50 명의 환자를 대상으로 Neurometer® CPT를 이용하여 수술 전으로 삼차 신경의 상악 분지에서 삼차 신경 손상으로 인한 안면부 감각 저하 여부 및 정도를 확인 하고, 수술 후 1 주일, 1 개월, 3 개월 및 6 개월, 이후로 3 개월 단위로 같은 검사를 시행 하여, 정상 소견이 나올 때까지 추적 하였다. 50 명 중 27 명은 술 전, 술 후 1 주일 모두 정상치가 나와 삼차 신경 손상이 없는 것으로 보고 회복 기간 평가에서 제외하였다. 이상 소견이 있던 23 명의 평균 삼차 신경 회복 기간은 1.46 개월이었다. 골절 종류 및 관골 골절의 아군, 안와골 골절의 아군 간 회복 기간 차는 통계학적으로 유의하지 않았다. 기존에 Neurometer® CPT를 이용하여 다양한 말초 신경병증을 평가하는 연구는 많이 있었으나, 이를 이용하여 회복 기간을 평가하는 연구는 없는 점에 대해, 본 연구는 회복 기간을 평가함으로써 그 의의가 크다고 할 수 있다. 삼차 신경 손상이 있던 안면골 골절 환자들을 대상으로 Neurometer® CPT를 이용하여 안면 감각 회복 기간을 객관적으로 평가 할 수 있었고, 예상외로 빠른 회복 기간을 보였다.

Facial hypoesthesia is one of the most cumbersome complaints in the management of facial bone fractures(fracture of zygoma, blow out fracture). But there is lack of literature dealing with facial sensory recovery after managing the fractures. The purpose of this study was to retrospectively evaluate facial sensory recovery periods in facial bone fractures accompanying trigeminal nerve injuries using Neurometer® CPT. Fifty facial bone patients who underwent open reduction in our institute from December 2013 to March 2015 were included in the study. Patient’s facial sensory status were repeatedly checked at 1 week, 1 month, 3 months and 6 months postoperatively until the results were normalized. Among the subjects, 27 patients were excluded since preoperative and postoperative Neurometer® CPT results were normal. In general, all patients required 1.46 month on average to recover. There was no statistically significant difference of sensory recovery period between fracture types, subtypes of zygoma fracture and subtypes of blow out fracture. This study is first study to evaluate facial sensory recovery period. Facial sensory recovery period of facial bone fracture patients could be evaluated objectively with Neurometer® CPT, and the recovery period was quite faster than we expected.
