건강 검진 수검자에서 혈청 Irisin 수치와 비알코올성 지방간 간의 관계

Abstract

Irisin is a recently found myokine that aids obesity control and improves glucose homeostasis by acting on white adipose tissue cells and increasing total energy consumption. The aim of this study was to evaluate serum irisin levels in patients with non-alcoholic fatty liver disease (NAFLD) and to compare these levels with those of normal controls. Among 595 health screen examinees who had visited our institute between January 2013 to and March 2013, 355 patients (84 NAFLD patients and 271 normal controls) were enrolled depending on whether they gave written informed consents and their history of alcohol intake, blood tests, and ultrasonographic findings. Age; sex; laboratory test parameters; homeostasis model assessment-insulin resistance; and levels of leptin, adiponectin, and irisin were assessed. Serum irisin levels (ng/ml) were significantly higher in the NAFLD group than in the normal controls and higher in the mild fatty liver group than in the moderate-to-severe fatty liver group. Serum irisin levels were significantly higher in NAFLD patients, which is not consistent with the results of previously published studies. Therefore, more studies are needed to confirm the role of irisin in NAFLD.

Irisin은 최근 새롭게 발견된 근육 호르몬 물질로, 백색 지방 조직 세포에 작용하여 비만 조절을 돕고 포도당 항상성을 향상시켜 전체 에너지 소비를 증가시킨다. 이 연구의 목적은 비알코올성 지방간 환자들에서 혈청 Irisin 수치를 평가하고 이를 대조군과 비교하는 것이다. 2013년 3월부터 5월까지 본원 건강 검진 센터를 방문한 수검자 595명 중 동의서를 작성한 355명의 환자를 등록하였고 이 중 84명은 비알코올성 지방간 환자, 271명은 건강한 대조군이었다. 이들을 대상으로 나이, 성별, 혈청 검사, 인슐린 저항성, Leptin, Adiponectin, Irisin 수치들을 측정하였다. 혈청 Irisin 수치는 대조군에 비해 비알코올성 지방간 환자에서 더 높게 나타났으며, 경도의 지방간 그룹에서 중등도 및 중증의 지방간 그룹보다 더 높게 나타났다. 혈청 Irisin이 비알코올성 지방간 환자에서 더 높게 측정된 점은 이전까지 발표된 연구 결과와는 일치하지 않으며, 따라서 비알코올성 지방간 환자에서 Irisin의 역할에 대해서 추가적인 연구가 필요할 것이다.
