구속형 폴리에틸렌 라이너를 이용한 인공 고관절 재치환술

Abstract

인공 고관절 전치환술 후 재발성 탈구 및 탈구의 고 위험인자를 가진 환자들에 대해 구속형 폴리에틸렌 라이너를 이용하여 인공 고관절 재치환술 을 시행하고 그 결과를 알아보고자 하였다. 2002 년 1 월부터 2012 년 5 월까지 구속형 폴리에틸렌 라이너를 이용하여 재치환술을 시행하였던 24예의 환자 중 2 년 이상 추시 관찰이 가능하였던 21 예의 환자를 대상으로 하였다. 추시 기간은 평균 5.1 년이었고 남자가 9 예, 여자가 12 예였다. 재치환술 당시 평균 연령은 72.4 세, 재치환술 전 평균 탈구는 3.0 회였다. 최종 추시까지 구속형 폴리에틸렌 라이너 실패의 발생 빈도 및 탈구 위험인자와의 상관관계를 조사하였다. 구속형 라이너의 실패는 4 예에서 발생하였으며 탈구가 2 예, 비구컵 이완이 2 예였다. Harris 고관절 점수는 술 전 평균 38.0 점에서 술 후 평균 73.9 점으로 향상되었다. 재치환술시 폴리에틸렌 라이너만 교체하였던 4 예 중 3 예에서 삽입물의 실패가 발생하여 비구컵과 라이너를 모두 교체하였던 환자들에 비하여 유의하게 높은 실패율을 보였으나 삽입 방법에 따른 차이는 없었다. 구속형 폴리에틸렌 라이너는 재발성 탈구 및 탈구의 고 위험인자를 가진 환자에서 선택할 수 있는 좋은 치료 방법 중 하나이나 본 연구에서 비교적 높은 임상적 실패율을 보여 이에 대한 주의가 필요할 것으로 사료된다.

This study evaluated clinical and radiographic results of revision total hip arthroplasty (THA) with constrained polyethylene liner for the patients who underwent recurrent dislocations after THA and had risk factors of dislocation. 21 patients who underwent revision THA with constrained polyethylene liner from January 2002 to May 2012, with had at least 2 years of follow-up were studied. There were 9 men and 12 women. The mean follow-up period was 5.1 years and mean patient age was 72.4 years, the mean number of dislocation was 3.0 times. The incidence of the constrained polyethylene liner failure and the correlation between the failure and risk factors of dislocation was analyzed. There were 4 failures, and 2 dislocations, and 2 cases of acetabular cup loosening. The mean Harris hip score was improved from 38.0 preoperatively to 73.9 postoperatively. Among the 4 cases polyethylene liner changed only at the time of revision THA, 3 cases failed. The failure rate was higher than it of the group who were changed both acetabular cup and polyethylene liner. There weren’t significant differences according to fixation methods. Revision THA with constrained polyethylene liner is a good treatment option. But, relatively high clinical failure rate should be concerned.
