이개 손상이 동반된 불안정성 족관절 골절에서 관통 나사 고정 후 제거에 따른 경과

Abstract

이개 손상이 동반된 불안정성 족관절 골절에서 관통나사는 빈번하게 사용되고 있다. 본 연구의 목적은 족관절 골절에서 관통 나사의 잔존 및 제거에 따른 임상적인 평가와 방사선학적 결과에 끼치는 영향에 대하여 알아보고자 한다. 불안정 족관절 골절 후 관통 나사의 고정이 필요하였던 29예의 환자를 대상으로 하였다. 14예에서는 체중 부하 전 관통 나사를 제거 하였으며, 15예에서는 관통 나사를 제거 하지 않았다. 임상적인 평가는 Bray’s 평가 점수를 이용하였으며, 방사선학적 평가는 경비 간격과 경비 중복 간격을 측정하여 비교하였다. 임상적인 결과는 관통 나사를 남겨 둔 그룹에 비하여 제거한 그룹에서 더 좋은 것으로 나타났다. 관통 나사를 남겨 둔 그룹의 Bray’s 평가 점수는 91.67점이었고 이와 비교하여 제거한 그룹에서는 76.43이였다. 반면에 방사선학적 결과에서는 통계학적인 유의성은 없었다. 관통 나사 제거 그룹은 7명에서 이개의 재발이 관찰되었으나 관통 나사를 남겨둔 그룹에서는 1명의 재발이 관찰되었다. 관통 나사를 남겨둔 그룹은 제거한 그룹에 비하여 더 좋은 임상적인 결과를 보였다. 그러나 방사선학적인 결과에서는 두 그룹에서 통계학적 유의성은 없었다. 그러나 관통 나사를 제거한 그룹에서 이개 재발의 위험성이 더 높게 나타났다.

Syndesmotic screws are frequently placed in unstable ankle fractures with syndesmotic injury. The purpose of this study is to determine whether clinical outcomes and radiographic results after ankle fracture are affected by the retention or removal of the syndesmotic screw. An analysis was conducted on 29 patients, with an unstable ankle fractures requiring syndesmotic screw fixation. The syndesmotic screw was removed prior to weight bearing in 14 patients and retained in 15 patients. Clinical outcomes were measured using the Bray’s scoring system, and radiologic results included tibiofibular clear space (TFCS) and tibiofibular overlap (TFOL). Clinical outcomes were improved in patients with removed screws compared with those with retained screws. The Bray’s scoring system for patients with retained screws was 91.67 compared with 76.43 for the removed group. Otherwise radiologic results were not statistically significant. 7 patients in the removed screw group had recurred syndesmotic diastasis, but only one patient in the retained group had this recurrence. A retained screw was associated with a better clinical outcomes compared with a removed screw. However, there was no significant difference in radiologic results between the two groups, but the risk of recurrence increased in the removed screw group.
