초음파를 통한 갑상선 유두암의 피막 외 확인을 위한 지표

갑상선 유두암은 가장 흔한 갑상선 암으로 최근 초음파의 발달에 따라 진단율이 높아지고 있다. 수술적 치료로는 갑상선 전 절제술, 일엽 절제술 등이 시행되고 있는데 그 크기가 1 cm 미만일 경우에는 전절제술을 피하는 추세이다. 그러나 피막 외 침범이 있을 경우에는 전절제술과 술 후 요오드방사선 치료가 권고된다. 이에 술 전 초음파를 통해 피막 외 침범을 확인하고 치료방법을 정하는 것이 필요하다. 본 연구에서는 술 후 병리적 피막 외 침범을 진단 받은 환자의 술 전 초음파를 분석하여 피막 외 침범을 나타내는 지표를 알아보고자 한다. 2005년 1월부터 2013년 12월까지 갑상선 유두암을 진단받고 수술을 시행한 환자 중 본원 영상의학과 전문의에게 술 전 초음파 검사를 받은 265예를 대상으로 술 전 갑상선 초음파를 재검토하여 종양의 크기, 임파선 전이, 석회화, 종양 실질 양상, 피막 접합 유무, 피막 접합 정도를 확인하고 술 후 병리적 피막 외 침범 결과와 비교하여 점수화 하였다. 총 265예 중 남자 47명 여자 218명으로 여자의 수가 많았으며 병리적 피막 외 침범이 있는 경우는 123예, 없는 경우는 142예였다. 초음파 인자들 중 석회화와 종양실질의 양상은 피막 외 침범과 연관성이 없는 것으로 나타났고, 종양의 크기, 임파선 전이, 피막 접합 유무, 피막접합정도는 피막외침범과 연관성이 있는 것으로 나타났다. 초음파 소견에 가중치를 주어 점수화하여 0점에서 10점으로 나누었으며 0-2점은 16.5%, 3-5점은 74.5%, 6-8점은 81.3%, 9점 이상은 100% 피막 외 침범에 나타났다.

Extrathyroidal extension which increases the risk of morbidity and mortality is the deciding factor in surgical extension of papillary thyroid carcinoma. Ultrasonography is the most exact tool for detecting extrathyroidal extension. However, there are few studies on pre-opreative ultrasonographic findings about extrathyroidal extension. The purpose of reserch was to evaluate the predictive sonographic index of extrathyroidal extension. 265 patients with papillary thyroid carcinoma who underwent preoperative sonography and surgery were respectively reviewed. Sonographic findings were compared with pathologic results. There were no differences in calcification and echo texture. However, there were significant differences in terms of mass size, lymph node metastasis, capsule contact, and degree of capsule contact. We gave score to these 4 factors and defined extrathyroidal index as total score. Scoring had high sensitivity, specificity and predictability. We suggest the extrathyroidal extension index with sonography is hepful for preopeative sonographic diagnosis.
