Matriderm® 이용한 당뇨병성 족부궤양의 치료

Abstract

당뇨병성 족부궤양은 신경 및 혈관의 동반 손상, 창상 치유 과정의 결함 등으로 인해 보존적 또는 수술적 치료에 제한점이 많다. 이에 저자는 최근 조직공학기술의 비약적 발전을 통해 피부 이식술의 단점을 극복하고자 개발된 인공 진피 중 하나인 메트리덤^(R)(Matriderm^(R); Skin Health, Billerbeck, Germany)을 피부 이식술시 함께 사용하여, 당뇨병성 족부궤양의 치료에 있어 새로운 치료 방법을 모색해 보았다. 본원에서 당뇨병성 족부궤양으로 치료받은 60명의 환자를 대상으로 하였다. 평균나이는 58세였으며, 궤양이 생긴지 4주가 지난 만성궤양을 가진 제 2형 당뇨환자였으며, 감염 및 혈관병증의 증거가 있는 경우는 제외하였다. 실험군과 대조군은 무작위 배정하였으며, 실험군은 메트리덤^(R)과 부분층 피부 이식술을 동시에 시행하였고, 대조군은 부분층 피부 이식술만 시행하였다. 평균 입원 기간은 실험군에서 7.52주, 대조군에서 9.22주로 조사되었으며(p < 0.05), 완치까지 소요된 기간은 실험군에서 8.61주, 대조군에서 12.94주로 조사되었다(p < 0.05). 술 후 12주에 조사한 완치 환자는 실험군에서 28명(93.3%), 대조군에서 24명(80.0%)으로 조사되었으며(p < 0.05), 건측에 대한 환측의 탄력도비는 실험군에서 0.72, 대조군에서 0.19로 조사되었다(p < 0.01). 당뇨병성 족부궤양의 치료에서, 메트리덤^(R)과 피부 이식술을 같이 시행한 군에서 상처 치유가 빠르고 재원 기간이 짧았으며, 탄력도도 높게 측정되어, 효과적인 당뇨병성 족부궤양의 치료를 기대할 수 있다.

Treatment of diabetic foot ulcers has many limitations for patients with neuropathy, vasculopathy, and impairment of wound healing. Many dermal analogues have been invented to overcome these limitations. Matriderm^(R), one dermal analogue, is made from bovine collagen and elastin. This study was undertaken to evaluate the effectiveness of Matriderm^(R) on diabetic foot ulcers in comparison to skin grafts. Sixty patients with diabetic foot ulcers were included in the study. The patients were on average 58 years-old and had type II diabetes mellitus. The patients were randomly selected into an experimental group and a control group. The experimental group had Matriderm^(R) appliance and split-thickness skin grafts while the control group only received split-thickness skin grafts. The experimental group had a shorter hospitalization period (7.52 weeks) than the control group (9.22 weeks), and required a shorter time (8.61 weeks) for complete healing than the control group (12.94 weeks) with a statistical significance (p < 0.05). The experimental group also had a higher elasticity ratio of the affected side to the non-affected side than the control group (p < 0.01). These results suggest that Matriderm?? enables effective healing and improves elasticity in patients with diabetic foot ulcers.
