투명 신세포암에서 Carbonic Anhydrase IX의 발현과 예후와의 관계

Abstract

최근 투명 신세포암에서 Carbonic anhydrase IX(CAIX) 발현과 예후 예측의 효용성에 대한 많은 연구가 보고 되고 있으며 이에 투명 신세포암에서 CAIX 발현과 임상적 예후 예측인자와의 관계에 대하여 연구하였다. 1999년 1월부터 2007년 12월까지 본원에서 신절제술 이후 신세포암으로 진단받은 172명의 의무기록을 후향적으로 분석하였다. 신세포암의 아형과 CAIX발현의 관계 및 투명 신세포암으로 진단받은 141명 중 CAIX 발현 정도에 따라 85%이상 발현된 106례를 고발현군으로 85% 이하 발현된 35례를 저발현군으로 분류하여 분석하였다. 투명 신세포암과 비투명 세포암에서의 CAIX 발현 비교에서 고발현된 투명 신세포암은 106례(97.2%), 비투명 신세포암은 3례(2.8%), 저발현된 투명 신세포암은 35례(55.6%), 비투명 신세포암은 28례(44.4%)로 고발현된 투명 신세포암이 많았다(p < 0.01). 나이, 체질량 지수, 술전 M병기, 핵분화도, 술후 전이 여부, 종양 특이 생존률은 두 군간의 차이가 없었다(p = 0.237, p = 0.576, p = 0.257, p = 0.802, p = 0.382, p = 0.551) 그러나 낮은 CAIX발현에서 술전 T, N 병기가 높았다 (p = 0.015, p = 0.033). 신세포암 중 투명 신세포암에서 CAIX이 고발현 되었고 낮은 CAIX발현에서 술전 T, N병기가 높게 나타났다. 따라서 CAIX는 신세포암에서 투명 신세포암의 구분 및 투명 신세포암의 예후 예측에 도움을 줄 것으로 생각된다.

Reccently studies of Carbonic anhydrase IX (CAIX) expression and clinical significance in renal cell carcinoma (RCC) have given rise to disagreements in the usefulness of CAIX as a prognostic factor. The purpose of this study was to evaluate the association between CAIX expression and clinical factors in RCC. The medical record of 172 RCC patients in our hospital (from January 2000 and December 2007) were reviewed retrospectively. Patients were divided into a high expression group (109 cases) and a low expression group (63 cases) according to their CAIX expression. We evaluated the association between CAIX expression and age, body mass index (BMI), renal neoplasm type, tumor stage, nuclear grade, metastasis after surgery and tumor-specific survival rate. The mean ages of the high expression group and the low expression group was 56 years and 54 years respectively. The mean BMIs of the high expression group and the low expression group were 24.2 kg/m2 and 24.5 kg/m2 respectively. Considering the difference between clear cell RCC and non clear cell RCC, CAIX was significantly expressed in clear cell RCC. There was no significant difference between high expression clear cell RCC and low expression clear cell RCC according to age, BMI, nuclear grade, metastasis after surgery and tumor-specific survival rate (p = 0.237, p = 0.802, p = 0.382, p = 0.551), but in clear cell RCC, CAIX expression was significantly expressed in patients with higher T or N stages (p = 0.015, p = 0.033) CAIX was significantly expressed in clear cell RCC and was significantly lower expressed in patients with higher T stage or N stage.
