비측 미숙아망막병증의 고찰

Abstract

본 연구는 비측 미숙아망막병증의 발생학적 임상양상과 원인에 대해 알아보고자 하였다. 2004년 1월부터 2009년 12월까지 동산병원을 방문하여 레이저 광응고술을 받은 미숙아망막병증을 가진 56명의 111눈을 대상으로 하였고, 태생 시 및 첫 진단 시 재태주수와 발생 위치, 레이저 치료 시점과 횟수에 대해 연구하였다. 비측 미숙아망막증을 가진 환아는 33눈(29.7%)이었고, 비측 미숙아망막병증 환아의 평균 재태주수는 이측 미숙아망막병증 환아의 평균 재태주수보다 빨랐다. 레이저 치료를 받지 않은 모든 비측 미숙아망막병증 환아는 몇 주안에 모두 비측보다 이측이 더욱 심한 병변으로 진행하였다. 레이저 광응고술을 받은 평균 재태주수도 비측 미숙아망막병증에서 빨랐으며, 레이저 광응고술의 평균 시행횟수는 비측 미숙아망막병증에서 더 많았다. 그리고 안저검사상 비측의 무혈관부위가 이측보다 넒은 환아는 없었다. 본 연구에서 비측 미숙아망막병증은 비전형적으로 비측 무혈관부위가 이측보다 넓기 때문이 아니라 비측에서 미숙아망막병증이 더 일찍 발생하기 때문이고, 이 경우에 더욱 심한 병의 경과를 가진다는 것을 알 수 있었다.

We studied the developmental characteristics and causes of the nasal retinopathy of prematurity (ROP). We examined 111 eyes in 56 ROP-affected infants who had been treated with laser photocoagulation by retina specialists at Dongsan hospital from January 2004 to December 2009. We analyzed the gestational age at birth and diagnosis, location of lesion, time and number of laser photocoagulation therapy. Thirty three cases (29.7%) were nasal ROP. Mean gestational age at birth of nasal ROP was lower than that of temporal ROP. All the cases of nasal ROP without laser treatment developed more severe temporal lesions rather than nasal lesions within several weeks. Also, the average gestational age of infants who received laser photocoagulation was lower, and the average number of trials of laser photocoagulation of ROP was more in nasal ROP than in temporal. There was no case with wider avascular area in nasal retina than temporal. In nasal ROP, nasal lesion develops earlier than temporal one, even though temporal lesion develops more severe course than nasal area in several weeks. The nasal ROP was appeared because ROP lesion developed earlier in nasal retina than temporal, not because there was any wider nasal avascular area atypically.
