과체중 및 비만한 성인 남성에서 체중력과 지방간과의 관련성

Abstract

연구배경: 비만은 비알코올성 지방간 질환의 주요 위험인자이다. 그리고 체중감소는 비만한 지방간 환자에서 지방간을 호전시킨다. 최근 비만하지 않은 남성에서의 체중력(체중변화)은 지방간발생의 주요인자라는 것이 밝혀져 있다. 그러나 과체중이나 비만한 남성에서의 체중력과 지방간 발생과의 관계는 알려진 바가 없는 실정이다. 이에 본 연구자는 과체중 또는 비만인 성인 남자에서 체중력과 지방간의 관련성을 조사하고자 이 연구를 진행하였다. 연구방법: 대구광역시 일개 병원의 건강검진센터를 방문한 30~59세 사이의 성인 남자 중 과체중 또는 비만이면서 지방간이 있는 107명과 체질량지수가 비슷하지만 지방간이 없는 107명을 대상으로 현재 체중, 현재 신장을 측정하고, 20대 초반 체중, 생애 최고 체중과 연도에 대한 설문조사를 시행하였으며, 개개인의 현재 BMI, 20대 초반 BMI, 생애 최고 BMI와 그 변화량을 계산하여 지방간 유무와의 연관성을 통계적 분석을 통해 조사하였다. 연구결과: 1) 대상자의 평균 연령은 41세, 평균 BMI는 25.5 kg/m2이었다. 2) 20대 초반 BMI는 지방간이 없는 군에서 더 높았다. 3) 비만한 성인남성에서 20대 초반 BMI에 비한 현재 BMI의 증가량은 지방간 군에서 유의하게 높았다. 4) 비만한 성인 남성에서 생애 최고 BMI에 비한 현재 BMI의 감소량은 정상간 군에서 유의하게 높았다. 결론: 본 연구결과 현재 비만한 성인 남성에서 20대에 비한 BMI 증가정도는 정상간 군에서 지방간 군에 비해 유의하게 적었으며 초기 성인기의 BMI를 최대한 높아지지 않게 유지하는 것이 지방간의 유병율을 낮출 수 있을 것으로 기대된다.

Background: Obesity is a major risk factor of non-alcoholic fatty liver diseases. Weight control is associated with the improvement of fatty liver in obese patient. However, whether history of weight change affects the occurrence of fatty liver has not been evaluated in obese patients. The purpose of this study was to investigate the relationship between individual weight change history and current the presence of fatty liver in adult male with elevated body mass index (BMI). Methods: Survey was performed to compareI the early 20s' BMI, the highest BMI of life (HBL) and the recent BMI from 107 cases that have been found with fatty liver by ultrasonography, as well as 107 controls with similar BMI, but without fatty liver. Results: The difference between early 20s' BMI and recent BMI was greater in fatty liver group than control group. The decreasing rate of HBL to recent BMI was less in fatty liver group than control group. Conclusion: There was significant association between the increasing rate of BMI from early twenties and prevalence of fatty liver. Hence, it may be expected that prevention of weight gain is critical to prevent fatty liver even in obese patients.
