폐경 전 여성에서 골밀도와 심혈관계 위험인자와의 관련성

Abstract

골다공증은 인구의 노령화와 함께 증가하고 있고 사망률과 이환율을 높이는 중요한 원인이 되고 있다. 최근 골다공증을 가진 환자들에서 심혈관 질환이 증가하고 있다는 보고가 있었고 본 연구에서는 폐경 전 여성들을 대상으로 골밀도와 심혈관계 위험인자와의 관련성을 밝히고자 하였다. 단일 대학병원 건강증진센터를 방문한 폐경 전 여성 총 379명을 대상으로 단면연구를 시행하였다. 이중 에너지 방사선 흡수계측법(DXA) 을 이용하여 요추부 골밀도를 측정하였고 신체 계측 및 혈액 검사가 수행되었다. 심혈관계 위험인자인 허리둘레, 혈압, 공복혈당, 당화혈색소, 혈중 지질 등과 요추 골밀도와의 상관관계를 조사하였다. 나이와 체질량지수를 보정한 후 요추 BMC는 이완기 혈압, 총콜레스테롤, 중성지방, 저밀도지단백 콜레스테롤과 음의 상관관계를 보였고 요추 BMD는 허리둘레, 이완기 혈압, 당화혈색소, 중성지방과 음의 상관관계를 보였다. 폐경 전 여성에서 골밀도는 심혈관계 위험인자인 비만, 혈압, 지질과 관련성이 있으므로 골감소가 진행되는 초기에 심혈관계 질환을 함께 고려한 생활습관과 식생활 개선이 이루어진다면 심혈관계 질환의 이환율과 사망률 감소에 도움이 될 뿐만 아니라 삶의 질이 개선될 것이다.

Osteoporosis is a major cause of morbidity and mortality with an increasingly elderly persons. Recent studies have shown higher prevalence of cardiovascular disease among patients with osteoporosis. The purpose of this study was to determine the relationship between bone mass and cardiovascular risk factors in premenopausal women. We conducted a cross-sectional study of 379 premenopausal women who had visited the health promotion center. Bone mineral density (BMD) of lumbar spine was measured using dual energy X-ray absorptiometry (DXA). Physical examination and laboratory tests were performed. We evaluated the relationship between bone mineral content (BMC) or BMD of lumbar spine and cardiovascular risk factors. After adjusting for age and body mass index (BMI), BMC of lumbar spine was negatively correlated with diastolic blood pressure (BP) and serum lipids and BMD of lumbar spine was negatively correlated with waist circumference, diastolic BP, hemoglobin A1c (Hb A1c), and triglycerides (TG). These findings suggest that bone mass is associated with cardiovascular risk factors.
