굴절조절내사시에서 안경착용 후 임상특징의 변화

Abstract

굴절조절내사시에서 굴절이상 교정이 임상양상과 치료결과에 미치는 영향에 대해 알아보고자 하였다. 굴절조절내사시환자 60명을 대상으로 주증상, 발생시기, 안경착용시기, 굴절이상, 안경 착용 전후의 시력, 사시각, 입체시 등을 조사하였다. 초진시 연령은 평균 8.50 ± 3.87세였고, 병력에 의한 발생시기는 평균 30.70 ± 15.74개월, 첫 안경 착용시기는 평균 40.60 ± 15.26개월이었다. 추적관찰 기간은 평균 59.20 ± 40.26개월이었다. 교정전 원시는 평균 4.90 ± 1.75 D였고, 원시 정도의 변화는 53명에서 평균 0.94 ± 0.91 D감소하였고 7명에서는 평균 0.38 ± 0.17 D증가하였다. 사시각은 초진시 안경착용 전 원거리에서 24.4 ± 9.10 PD, 근거리에서 28.4 ± 9.05 PD에서 최종방문시 안경착용 후 원거리에서 1.50 ± 2.56 PD, 근거리에서 3.20 ± 3.50 PD로 감소하였다. 전교정안경의 착용과 가림치료를 통해 초진 시에 비해 최종 방문시 약시의 빈도가 감소하였다. 그러나 굴절부등 동반, 항상내사시의 경우는 약시의 발생과 연관될 수 있으므로 가림치료 등의 적극적인 치료가 필요하다. This study aimed to evaluate the effect of refractive error management on the clinical features and resolution of accommodative esotropia. We retrospectively reviewed the charts of 60 patients, including chief complaint at first visit, age of onset, initiation of spactacles, cycloplegic refraction, visual acuity with and without spactales, angle of deviation, stereoacuity. Mean age at first visit was 8.50 ± 3.87 years, mean age of onset was 30.70 ± 15.74 months, and mean age of initiation of spactacles was 40.60 ± 15.26 months. Mean follow-up was 59.20 ± 40.26 months. Mean initial cycloplegic refraction was 4.90 ± 1.75 D (diopters). Fifty-three patients had decreased hypermetropia, averge of 0.94 ± 0.91 D, while 7 patients had increased hypermetropia of averge 0.38 ± 0.17 D. Mean deviation at initial visit was 24.40 ± 9.10 PD (prism diopters) at distance, 28.40 ± 9.05 PD at near without glasses. And decreased to 1.50 ± 2.56 PD at distance and 3.20 ± 3.50 PD at near with glasses at final visit. Amblyopia was reduced with spectacles and patching. Anisometropia, constant misalignment were found to be a significant risk factor for the development of amblyopia in accommodative refractive esotropia. Therefore, such children must receive careful treatment.
