종골의 관절 내 분쇄 골절에서 다발성 핀 고정술의 효과

Abstract

종골 골절은 조기에 수술적 치료가 필요한 골절이지만 그 결과가 양호하지 못하고 합병증이 많이 발생하여 치료에 대한 논란의 여지가 많은 골절이다. 최근에 관혈적 정복 후 금속판 내고정술을 많이 사용하고 있지만 내, 외측 피질골의 분쇄가 심한 골절에서 충분한 고정력을 가지지 못하는 경우가 많고, 금속판의 두께 때문에 피부 자극을 유발할 수 있는 문제가 있다. 따라서 거골하 관절을 침범한 종골 분쇄 골절의 관혈적 정복 및 내고정술 치료 시 다발성 핀 고정술의 효용성에 대하여 알아보고자 하였다. 거골하 관절면 및 내, 외측 피질골의 분쇄가 심한 종골 골절에서 관혈적 정복 시 금속판 고정술이 힘들다고 판단되어 다발성 핀을 이용한 고정술을 시행하고, 최소 1년 이상 추시가 가능하였던 21명, 24예를 대상으로 하였다. 단순 방사선 사진을 이용하여 수술 전, 수술 직후, 최종 추시 시의 융기관절각, 십자각, 종골의 높이와 길이의 비를 측정하였고 변화를 비교 분석하였다. 결과적으로 종골의 융기관절각, 십자각, 종골의 높이와 길이의 비에서 수술 전과 비교하여 수술 직후에 해부학적으로 만족할 만한 정복을 얻었으며, 수술 직후부터 최종 추시 사이의 방사선학적 계측에서는 통계학적으로 변화가 없어 (P>0.05) 다발성 핀 고정 방법이 골절 정복과 유지에 효과가 있다고 판단되었다. 주요 합병증으로는 거골하 관절염 2예, 충돌 증후군 1예, 비복 신경 손상 1예, 피부 봉합부의 부분적인 피부 괴사 2예, 만성 골수염 1예가 있었으나 치명적인 합병증은 없었다. 그러므로 거골하 관절을 침범한 종골 분쇄 골절에서 시행한 다발성 핀 고정술은 방사선학적으로 충분한 고정력이 있으며 효과적인 고정 방법 중 하나로 사료된다. The purpose of this study is to evaluate the efficacy of open reduction and multiple pin fixation in the treatment of intra-articular comminuted calcaneal fractures. We analysed retrospectively 24 intra-articular comminuted calcaneal fractures, which had undergone open reduction and multiple pin fixation because of impossibility of fixation with plate. As for radiographic parameters, we evaluated the B？hler angle, the Gissane angle, the height to length ratio of the calcaneus in preoperative, immediate postoperative and follow-up stage. Intra-articular comminuted calcaneal fractures were reduced satisfactorily in immediate postoperative stage. And the B？hler angle, the Gissane crucial angle, and the height to length ratio of the calcaneus resulted in no significant differences between immediate postoperative stage and last follow up stage (P>0.05). Complications included subtalar arthritis, impingement syndrome, sural nerve injury, skin necrosis of suture margin and chronic osteomyelitis. The results of this study showed that multiple pin fixation appeared to be radiologically a valid fixibility of treatment for intra-articular comminuted calcaneal fractures.
