특발성 황반원공에서 수술 후 시력결과의 형태학적 예측인자

Abstract

본 연구는 특발성 황반원공에서 수술 후 시력결과의 형태학적 예측인자에 대해 알아보고자 하였다. 특발성 황반원공으로 유리체절제술을 시행 받고 6개월 이상 추적관찰이 가능하였던 총 51명 51안을 대상으로 하였다. 술 후 시력이 20/50 미만인 28안을 1군, 20/50이상으로 유지되는 23안을 2군으로 나누고, 이들 중 황반두께가 정상범위로 복원된 총 33안을 다시 술 후 시력이 20/50미만인 15안을 A군, 20/50이상으로 유지되는 18안을 B군으로 나눈 뒤 술 후 시력예후에 관련된 여러 가지 술 전 및 술 후 인자를 분석하였다. 술 전 형태학적 인자로 1군에 비해 2군에서 기저직경과 최소직경이 유의하게 작았으며(p = 0.017, p = 0.000), 황반원공인자 및 원공견인지수(THI)는 유의하게 컸다(p = 0.002, p = 0.001). 술 후 인자로는 2군이 1군에 비해 황반윤곽이 좋았고 황반두께가 두꺼웠다(p = 0.048, p = 0.019). 술 후 황반두께가 정상 가까이 복원된 경우에서는 술 전 THI만이 A군에 비해 B군에서 유의하게 컸다(p = 0.013). 따라서 특발성 황반원공에서 빛간섭단층촬영으로 다양한 형태학적 인자들을 비교분석 하는 것은 황반원공 술 후 시력예후를 예측하는데 있어 의의가 있으며, 특히 술 전 THI가 크고, 술 후 황반윤곽이 좋은 경우 술 후 시력결과가 보다 양호할 것으로 추정된다. This study evaluated the morphological prognostic factors of postoperative visual outcome in idiopathic macular hole surgery. The records of 51 patients (51 eyes) who had undergone vitrectomy and followed up for at least 6 months were retrospectively reviewed. Patients with the postoperative visual acuity (VA) < 20/50 and with VA ≥ 20/50 were designated as group 1 and group 2 respectively. As a subgroup, patients with postoperative normal-ranged foveal thickness (33 eyes) were divided into A and B group which had the postoperative VA < 20/50 or ≥ 20/50 respectively. As the preoperative factors, group 2 comparing to group 1 had shorter base diameter and minimum diameter (p = 0.017, 0.000, respectively), and larger macular hole index and tractional hole index (THI) (p = 0.002, 0.001, respectively). As the postoperative factors, group 2 had better foveal contour and thicker fovea than in group 1 (p = 0.048, p = 0.019, respectively). In subgroup, only preoperative THI was significantly larger in group B (p = 0.013). This study showed that the various morphological factors using OCT, especially THI and foveal contour, seemed to be helpful in estimation of the functional outcome in macular hole surgery.
