대장내시경으로 인한 대장 천공의 수술적 및 비수술적 치료의 비교

Abstract

Background and Aim: Successful management of colonoscopic perforation by nonoperative treatment has been reported but management remains controversial. This study aimed to assess the prognostic factors and mortality due to colonoscopic perforations, and to compare operative and nonoperative management methods. Methods: Retrospective medical record review of colonoscopic perforations at five tertiary institutions between January 2000 and August 2006. Results: Forty-four patients were treated for colonic perforations following colonoscopy and the overall incidence was 0.06%. Eighteen patients were managed operatively and twenty？]six were managed nonoperatively. The mean hospital stay in the operative management group was longer, but the difference was not statistically significant (9.6 versus 14.3 days, p = 0.065). The only predictor of mortality was the status of bowel preparation (good, 0/39; poor, 3/5, p = 0.001). No statistically significant difference in mortality was observed between operative and nonoperative management (7.7% versus 5.56%, p = 1.000). onclusions: Our findings showed that the only predictor of mortality was the status of bowel preparation. Colonoscopic perforations can be successfully treated with either operative or nonoperative management in patients with adequate bowel preparation.

목적: 대장내시경 중 발생한 대장천공을 비수술적 방법으로 치료한 사례가 보고되어 왔지만 아직까지 논의의 여지가 있다. 본 연구에서는 대장내시경 중 발생한 대장천공에 있어 사망률과 예후인자를 분석하고 또한 수술적 치료와 비수술적 치료를 비교하였다. 방법: 2000년 1월부터 2006년 8월까지 5개 3차 의료기관에서 경험한 대장내시경으로 인한 대장천공의 의무기록을 후향적으로 분석하였다. 결과: 대장내시경중 44건의 대장천공을 경험하였으며 전체적인 발생률은 0.06%였다. 18명이 수술적 방법으로, 26명이 비수술적 방법으로 치료를 받았으며, 평균재원기간은 수술군에서 더 길었으나 통계적인 유의성은 없었다(9.6 versus 14.3 days, p = 0.065). 사망률과 관련된 유일한 예후인자는 장정결 상태였으며(good, 0/39; poor, 3/5, p = 0.001), 수술 또는 비수술군에 있어 사망률은 통계적으로 유의한 차이가 없었다(7.7% versus 5.56%, p = 1.000). 결론: 대장내시경 중 발생한 대장천공에 있어 사망률과 관련된 유일한 예후인자는 장정결 상태로 생각되며, 적절한 장정결 상태의 환자에서는 비수술적 방법으로도 수술적 치료에 못지않은 좋은 결과를 기대할 수 있을것으로 생각된다.
