대구광역시 및 경상북도 지역 초등학생들의 삼출성 중이염 유병율

Abstract

삼출성 중이염은 소아 이비인후과 영역의 질환 중 비교적 흔한 질환으로 4세까지 80%이상의 소아가 한번 이상 이환된다. 삼출성 중이염은 청력 장애를 일으키고 나아가 언어 습득과 학습 장애를 초래하게 된다. 그러나 우리나라에서 삼출성 중이염의 역학적 조사는 미흡하다. 특히 농촌 지역에 대한 유병율 조사는 아직 보고 된 바가 없는 실정이다. 이에 본원 난청연구소에서는 1999년 11월 부터 2001년 11월까지 대구광역시 및 경상북도 성주 및 청도 지역 초등학교를 선정하여 1-4학년 학생 3309명을 대상으로 삼출성 중이염 유병율을 조사 하였다. 삼출성 중이염은 이경(otoscope)을 통한 이학적 검사와 고막 운동성 계측 검사(Impedence audiometry), 그리고 통기 이경 검사(pneumatic otoscope)를 시행하여 진단되었다. 대구지역 초등학교 1-4학년 학생3225명 중 삼출성 중이염 환아는 165명으로 유병율은 5.12%였다. 경상북도 지역의 유병율은 3.06%였다. 도시지역인 대구지역 초등학생들의 삼출성 중이염 유병율이 농촌지역인 경상북도 지역에 비해 높게 나타났다. 또한 대구 및 경상북도 지역 모두 학년이 높아질수록 유병율이 낮아지는 경향을 보였다. 그 원인은 도시지역 어린이들이 집단생활에 노출 될 가능성이 높고, 환경오염 등의 환경적 요인과 또한 도시지역에서 모유 수유율이 낮은 사회 문화적 측면을 고려해야 할 것으로 생각된다. 그 외에 계절적 요인과, 사회 경제적 요인 등 여러 인자에 대한 추가적 연구의 필요성이 강조된다. Otitis media with effusion is a common disease in the field of otolaryngologic diseases, which affects 80% of children younger than 4 years of age least more than once. The purpose of this study was to determine the prevalence of otitis media with effusion (OME) in primary school children and the difference of the prevalence of OME in Daegu city and Gyungsangbookdo areas. The total 6535 children between 1st grade and 4th grade were included in the study. After otoscopic examination, the tympanogram and acoustic reflex tests were carried out. The prevalence rate of OME in primary school children in Daegu city was 5.12%, followed by 3.06% in Gyungsangbookdo. Our study showed a higher prevalence rate of OME in primary school children in Daegu city than those of Gyungsangbookdo and the higher the grade, the lower prevalence rate. The cause of higher prevalence rate of otitis media with effusion in Daegu city than Gyungsangbookdo may be contributed to the fact that the urban children are more exposed to living in a group. It is suggested that the further study of environmental and economical facts that have an impact on the prevalence rate of otitis media with effusion and an additional study of various factors are necessary.
