기본형 간헐외사시 환자에서 증량 양안 외직근후전술의 결과

Abstract

본 연구는 기본형 간헐외사시 환자를 대상으로 전통적인 양의 양안 외직근후전술과 증량한 양안 외직근후전술의 결과를 비교하고자 하였다. 대상 환자는 수술 후 6개월 이상 경과관찰 가능했던 전통적인 양의 양안 외직근후전술군 41명, 증량 양안 외직근후전술군 66명 총 107명을 대상으로 하였다. 수술양은 전통적인 양안 외직근후전술은 Parks공식을 따랐고, 증량 양안 외직근후전술은 한 눈당 사시각에 따라 전통적인 수술 양보다 1.5 mm 내지 2.5 mm 증량된 양을 사용하였다. 수술의 성공률은 최종 추적관찰시 전통적인 양안 외직근후전술은 43.9%, 증량 외직근후전술은 68.2%로 증량 양안 외직근후전술군에서 높았다(p=0.01). 과교정된 경우는 두 군에서 각각 1명씩 발생하여 전통적인 양안 외직근후전술은 2.4%, 증량 양안 외직근후전술군은 1.5%를 보였다. 부족교정된 경우는 전통적인 양안 외직근 후전술군은 53.7%, 증량 양안 외직근후전술군은 30.3%로 전통적인 양안 외직근후전술군에서 높은 부족교정율을 보였다(p=0.02). 이상으로 결과로 보아 기본형 간헐외사시 환자에서 양안 외직근후전술을 시행할 때는 전통적인 후전 양보다 한눈 당 1.5 mm 내지 2.5 mm 증량된 양의 후전술을 사용하는 것이 수술의 성공률을 높일 수 있는 방법으로 사료된다. This study was performed to compare the outcome of conventional (conventional surgery) and augmented (augmented surgery) amounts of symmetric lateral rectus recession in basic X(T) patients. The study population of 107 patients, who were followed up for more than 6 months, comprised 41 and 66 patients who underwent conventional and augmented surgery, respectively. The amount of recession in conventional surgery was according to Parks formula, whereas in augmented surgery, the augmented amount ranged from 1.5 mm to 2.5 mm per eye. At the last follow up, the surgical success rate of conventional and augmented surgery was 43.9% and 68.2%, respectively, which was significantly higher in the augmented surgery group (p=0.01). Over-correction occurred in 1 patient in each group, which was 2.4% and 1.5%, respectively. Under-correction occurred in 53.7% and 30.3% in conventional and augmented surgery, respectively, which was significantly higher in the conventional surgery group (p=0.02). The results suggest that augmenting the recession amount by 1.5 mm to 2.5 mm per eye, compared to the conventional recession amount, can improve the success rate when performing symmetric lateral rectus recession for basic X(T).
