수술환자 안전문제 해결을 위한 행위연구

Abstract

The purpose of the study was to describe the subjects’ participation experiences in action research program for the operating patients safety problems-solving, test the effectiveness of the program and provide a basis for promoting the active participation of Operating Room staff to the program. The mixed method design was adopted for the study. of it, quantitative research tests the effects of action program and qualitative research analyses the participants` experiences for this program. To solve the Operating Rooms safety problems, the action research was performed by Kemmis and McTaggart model, which consists of repeat performing the plan, implementation, observation and reflection process. The study results showed that participants reinforced the capability through the action research process for identifying surgical patients` safety issues, understanding the patient safety-related factors, planning the measures, applying the practical field and then reflecting the effectiveness and significance of the action research. In This study, the action program was established as a patient safety culture in Operating Room by checking and correcting the physicians` unattended time, enhancing the teamwork and improving communication strategy. In addition, it is considered that this action program produced improvement in patient safety-related surgical service conditions. This study explored process, effectiveness and significance of the action research in surgical patients` safety problems solving and investigated participants` the program experiences. first, surgical patients` safety problems solving process was performed by plan, implementation, observation and reflection sequence. second, the effectiveness of the program at the Operating Room field showed reduction in job error and shortening in physicians` unattended time. In participants` program experiences, the research participants identified operating-related safety issues in person by analyzing the causes, appling the action in operation, observing the process and effectiveness, and then reflecting the results. thus, there was a substantial improvement in participants` capability to the problem-solving. finally, the significances of the action research were awareness of the need to the action program for improving Operating Room conditions, participants` capacity-building and interest in the organization. In conclusion, the results indicated that surgical patients` safety issues were improved by the study participants` active and experiences in objective indicators and subjective data. The findings suggest a need for future research investigating the effectiveness of the action research for improvement of nursing care service and nursing staff`s job satisfaction in various care settings.

본 연구의 목적은 수술환자 안전문제 해결을 위한 행위연구를 실행하여 실행효과를 검증하고, 행위연구에 대한 연구 참여자들의 참여경험을 기술하고, 수술환자 안전문제 해결을 위한 실무자들의 적극적인 참여를 유발할 수 있는 접근법 마련의 기초자료를 제시하고자 하기 위함이다. 효과를 검증하기 위해서 양적자료를, 참여자의 경험을 규명하기 위해서는 질적자료를 수집하여 분석하는 방법론적 혼합설계이다. Kemmis와 McTaggart가 제시한 모형을 적용하여 수술환자 안전문제 해결을 위해 계획, 실행 및 관찰, 성찰의 단계를 반복하는 과정이다. 본 연구결과는 참여자들은 스스로 수술환자 안전문제를 확인하고 관련요인을 파악하고 실행안을 도출하여 실무에 적용하고 결과를 확인하고 성찰 하는 일련의 과정을 통해 참여자들 개인의 역량강화를 확인할 수 있었다. 참여자들이 의료인 부재 시간을 점검하고 이를 수정하여 교정함으로서 팀원들 사이의 팀워크 강화와 원활한 의사소통을 기반으로 수술환자안전을 위해 긴밀하게 밀착 되는 것을 확인한 바 수술실내 수술환자 안전문화로 정착되었다. 또한 수술환자 안전문제를 해결하기 위한 수행활동으로 수술업무의 개선을 확인하였다. 수술환자 안전문제 해결과정, 행위연구의 효과검증과 참여자 경험 측면에서 논의하였다. 수술환자 안전문제 해결 과정은 계획, 실행 및 관찰, 성찰의 과정으로 수행하였다. 행위연구 적용효과는 수술업무 오류의 감소, 의료인 부재시간의 감소로 양적효과를 확인하였다. 수술환자 안전문제를 스스로 탐색하고 적용함으로서 문제 해결 과정 경험의 질적 효과를 확인하였다. 본 행위연구의 의의는 참여자의 자발적이고 적극적인 업무분석과, 문제해결 능력을 개발과 간호서비스의 질적 개선을 통한 실무 환경의 변화, 참여자의 동기유발 효과 및 조직에 대한 관심도 향상에 기여하는 등 간호실무에 행위연구 적용 필요성의 제시에 대한 의의가 있다. 본 연구는 행위연구 참여자들의 적극적인 참여에 의해 수술환자 안전문제 해결을 위해 나타내는 객관적 지표의 개선과 질적 자료를 통한 참여자들의 경험으로 수술환자 안전문제 해결의 결과를 가져왔음을 알 수 있었다. 다양한 간호현장에서 간호 서비스 개선과 간호 인력의 직무만족 향상을 위해 행위 연구를 적용 하여 그 효과를 확인하는 연구를 제언한다.
