Assessment of Diabetic Using Current Perception Threshold

Abstract

I compared the current perception threshold (CPT) with nerve conduction study (NCS) to evaluate the usefulness of CPT for the diagnosis of diabetic polyneuropathy (DPN). I retrospectively enrolled patients with type 2 diabetes who underwent both CPT test and NCS from January 2013 to April 2016 and 142 patients were included in the final analysis. DPN is diagnosed by NCS. CPT was performed at index finger and great toe. Patients with a longer duration of diabetes and with symptoms tended to have a higher prevalence of DPN. In all frequencies (2000, 250, 5 Hz), CPT values in the DPN group were higher than the normal group at the lower limbs. After classified in normoesthesia or hypoesthesia group according to CPT, the subjects with DPN had a significantly higher prevalence of hypoesthesia than subjects without DPN. Only at the lower limbs, the receiver operating characteristics (ROC) curve analysis showed that CPT at all frequencies had a high area under curve (AUC) value for predicting the presence of DPN. In conclusion, CPT was clinically valuable in detecting nerve dysfunction of type 2 diabetes but a combination of NCS as a conventional method with CPT should be recommended for the diagnosis of diabetic polyneuropathy.

본 연구에서는 당뇨병성 말초신경병증을 진단함에 있어서 전류인지역치검사의 유용성을 평가하기 위해 신경전도검사와 비교를 시행하였다. 후향적 연구로, 2013년 1월부터 2016년 4월까지 제 2형 당뇨병 환자들 중 전류인지역치검사와 신경전도검사를 모두 시행한 142명의 환자들이 최종적으로 포함되었다. 당뇨병성 말초신경병증의 확진은 신경전도검사의 결과를 기준으로 하였다. 전류인지역치검사는 검지손가락과 엄지발가락에서 시행하였다. 당뇨병 이환기간이 길수록 증상이 있는 환자일수록 당뇨병성 말초신경병증의 유병률이 높았다. 전류인지역치검사의 모든 진동수(2000, 250, 5 Hz)에서 당뇨병성 말초신경병증으로 진단된 환자군이 정상군보다 측정값이 높았다. 대상 환자를 전류인지역치검사의 정상치를 기준으로 감각 저하군과 정상 감각군으로 분류했을 때, 당뇨병성 말초신경병증으로 진단된 환자군에서 감각저하환자의 유병률이 정상군에서보다 의미있게 높았다. Receiver operating characteristics (ROC) 곡선분석에서, 전류인지역치검사로 말초신경병증의 유무를 예측하는 데에 있어서 하지에서만 모든 진동수 값에서 높은 area under curve (AUC) 값을 가짐을 확인할 수 있었다. 결론적으로, 제2형 당뇨병에서, 전류인지역치검사는 신경의 기능이상을 찾아내는 데에는 의학적으로 가치를 지니지만 당뇨병성 말초신경병증을 확진함에 있어서는 반드시 기존의 검사법들을 병행하는 것이 추천된다.
