폐 동정맥 기형에서 Interlocking Detachable Coil 을 이용한 코일 색전술의 추적 결과 분석

Abstract

폐 동정맥 기형은 폐 동맥과 폐 정맥이 비정상적으로 정상적인 모세혈관 없이 직접적으로 연결되는 드문 기형이다. 이는 다양한 합병증을 일으키며, 특히 병변이 터졌을 경우 생명에 치명적인 폐 출혈을 초래하므로 반드시 치료가 필요한 질환이며, 코일 색전술과 같은 인터벤션 기법이 주로 시행되고 있다. 이 시술에는 다양한 죵류의 코일들이 사용되고 있으며, 그 중에서도 Interlocking Detachable Coil은 다른 코일과 비교하여 탈부착이 자유롭다는 장점이 있어 폐 동정맥 기형 치료에 널리 쓰이고 있으나 이에 대한 장기 추적 결과는 보고된 바가 적다. 따라서 본 연구에서는 Interlocking Detachable Coil을 이용한 폐 동정맥 기형 치료가 단기적으로 효과적일 뿐 아니라 장기적으로도 효과적이며 안전한 치료법임을 입증하고자 하였다. 폐 동정맥 기형으로 본원에 내원한 총 12 명의 환자를 대상으로 Interlocking Detachable Coil을 이용한 코일 색전술을 시행하였고, 추적 검사를 시행하여 결과를 분석하였다. 그 결과, 추적 검사 기간 중 시술과 관련된 합병증이 발생한 예는 없었으며, 단 한 사례에서 시술 6개월 후 부분적 재 개통이 확인되었다. 그러나 이 후 시행된 추적 검사 상에서 재 개통 된 병변의 크기에 변화가 없어 추가적인 시술은 시행되지 않았다. 그리고 이 외의 사례 에서는 재발은 보이지 않았다. 결론적으로, 이번 연구를 통하여 폐 동정맥 기형에서 Interlocking Detachable Coil을 이용한 코일 색전술이 효과적이고도 안전하며, 따라서 폐 동정맥 기형의 치료에 유용하게 이용될 수 있을 것으로 생각된다.

Pulmonary arteriovenous malformation (PAVM) is rare malformation in which the pulmonary artery and pulmonary vein are connected abnormally without normal capillaries. This is a disease that causes various complications, especially if the lesion is fatal and causes life-threatening pulmonary hemorrhage. In recent years, intervention techniques such as coil embolization have been performed. The Interlocking Detachable Coil (IDC) is widely used for the treatment of PAVM because it has advantage of detachment compared with other coils. However, long term follow-up results have not been reported. The purpose of this study was to prove that treatment of PAVM using IDC is not only effective in short term but also effective and safe in the long term. We performed coil embolization using IDCs in 12 patients with PAVM. We performed follow-up examinations. As a result, there were no complications related to the procedure. In only one case, partial recanalization was confirmed 6 months after the procedure. However, the recanalized lesion did not change in follow-up, therefore no additional procedures were performed. In other cases, no recurrence was seen. In conclusion, this study suggests that coil embolization using IDC in PAVM is effective and safe and may be useful for the treatment of PAVM.
