Urine Output and Biomarkers of Acute Kidney Injury in Very-Low-Birth-Weight Infants with Normal Serum Creatinine

Abstract

정상 혈청 creatinine(Cr) 농도를 보인 극소저체중아에서 소변량에 따른 소변 neutrophil gelatinase-associated lipocalin(NGAL) / creatinine(Cr) 비, 혈청 cystatin C(CysC), 혈청 Cr 수치를 비교하였다. 57명의 극소저체중아를 대상으로 후향적으로 48 시간 동안의 평균 소변량에 따라 소변 NGAL/Cr 비, 혈청 CysC 농도와 혈청 Cr 농도를 조사하였다. 5분 아프가 점수가 5점 미만이거나, 입원 중 혈청 Cr 농도가 1.5 mg/dL 초과한 경우, 혹은 동맥관 개존증으로 치료 받은 환아는 모두 제외하였다. 8시간 동안 소변량이 1.5 mL/kg/h 미만인 경우 핍뇨군으로 정의하였고, 114개의 검체 중, 25개(21.9%)의 검체가 핍뇨가 있을 때 측정되었다. 소변 NGAL/Cr 비와 혈청 Cr 농도는 대조군에 비해 핍뇨군에서 유의하게 높았다. 48 시간 동안의 평균 소변량은 NGAL/Cr 비와 혈청 CysC 농도와 음의 상관관계를 보였다. ROC curve에서 소변 NGAL/Cr 비는 혈청 Cr 농도나 혈청 CysC 농도 보다 핍뇨에 대한 큰 차별을 보였다. 결론적으로, 정상 혈청 Cr 농도를 보인 극소저체중아에서 소변 NGAL/Cr 비는 혈청 CysC와 혈청 Cr 농도 보다 급성 신손상의 예측에 더 민감한 인자로 보인다

We aimed to compare urinary neutrophil gelatinase-associated lipocalin (NGAL) / creatinine (Cr) ratio, serum cystatin C (CysC) levels, and serum Cr levels according to urine output in very-low-birth-weight infants (VLBWI) with normal serum Cr. Fifty-seven VLBWIs were enrolled. Urinary NGAL/Cr ratios, serum CysC concentration, and serum Cr concentrations were measured, and mean urine output for 48 h was determined. Infants with Apgar scores less than 5 at 5 min, serum Cr concentration greater than 1.5 mg/dL during hospitalization, or patients treated for patent ductus arteriosus were excluded. Infants with urine output of less than 1.5 mL/kg/h lasting more than 8 h were included in the oliguria group. Of 114 samples, 25 (21.9%) were collected during oliguria episodes. Urinary NGAL/Cr ratio and serum Cr concentration were significantly higher in the oliguria group than in the control group. Mean urine output for 48 h was negatively correlated with urinary NGAL/Cr ratio and serum CysC level. The urinary NGAL/Cr ratio demonstrated a greater discrimination than serum CysC and serum Cr concentrations. In conclusion, urinary NGAL/Cr ratio was more sensitive than serum CysC and serum Cr concentrations for detection of acute kidney injury according to urine output in VLBWIs with normal serum Cr.
