Effective Radiologic Parameters for Evaluation of Clubfoot Treatment

Abstract

선천성 만곡족의 평가를 위해 방사선학적 평가가 많이 이용되고 있으나, 어떠한 방사선학적 기준선이 치료 결과의 예측 및 평가에 있어서 객관적인지에 대해서는 아직 논란의 여지가 많다. 본 연구는 Ponseti 방법으로 치료한 환자들에서 수술적 치료의 필요성을 예측함과 선천성 만곡족의 전반적인 치료 결과를 분석함에 있어 방사선학적 기준선의 유용성을 찾고자 하였다. Ponseti 방법으로 치료를 시작한 환자 65 명(102 족)을 연구대상으로 하여 수술 여부에 따라 두 군(A군: Ponseti 방법을 통한 결과가 만족스러워, 수술을 시행하지 않은 군, B군: Ponseti 방법을 통한 결과가 충분하지 않아, 추가적으로 수술을 시행한 군)으로 나누었고, 두 군의 방사선학적 족부 변형 상태 비교를 위해 전후면 및 측면 거종간 각, 전후면 거골-제1중족골간 각, 측면 경종간 각을 측정했다. 73 족(A군)에서 Ponseti 방법만으로 치료 후 추가적인 수술 없이 성공적인 결과를 얻었고, 29 족(B군)에서 Ponseti 방법으로 치료 후 불만족스러운 결과로 추가적인 수술을 요하였다. 초기 방사선 소견상 측면 거종간 각 및 측면 경종간 각에서 두 군간 의미 있는 차이가 있었고, 전후면 거골-제1중족골간 각 및 측면 경종간 각이 선천성 만곡족 치료에 따른 변화에 통계학적으로 의미 있는 차이를 가졌다. 70% 이상의 환자에서 Ponseti 방법만으로도 성공적인 치료 결과를 얻을 수 있었다. 선천성 만곡족의 치료에서 방사선학적으로 초기 측면 거종간 각 및 측면 경종간 각을 통해 수술적 치료 여부를 예측할 수 있었고, 선천성 만곡족 치료 평가에 있어서 전후면 거골-제1중족골간 각 및 측면 경종간 각이 가장 객관적인 지표로 확인되었다.

The aim of this study was to identify which radiologic parameters have relevance to the predictability of surgical treatment and to the effect of clubfoot treatment. A total of 102 clubfeet were reviewed from 2005 to 2013. The patients were divided in two groups (Group A: patients who did not require additional surgery; Group B: patients who needed additional surgery). Radiographs were made at the first visit and the final follow up day for both two groups, and additional radiographs were made just before the additional surgery for group B. Four kinds of angle were separately measured by two orthopedic doctors, and the results were evaluated in combination. Group A was 73 clubfeet and Group B was 29 clubfeet. The result of a congenital clubfoot treated with the Ponseti method was successful in more than 70% of patients. The initial talo-calcaneal angle and tibio-calcaneal angle on lateral view were the radiologic parameters that can predict the need for additional surgical treatments. Talo-1st metatarsal angle on anteroposterior view and tibio-calcaneal angle on lateral view were able to effectively evaluate changes in clubfeet after treatment.
