45세 이후에 초발한 망상성 정신병의 특징(Ⅱ)

Abstract

요 약 저자는 1981년 1월부터 1986년 7월까지 계명대학교 의과대학 정신과에 입원한 환자로서 45세이후에 초발한 후기망상증환자 40명과 18명의 정신분열병 환자를 조사하였던바 후기망상증군과 정신분열병군 간에 통계적으로 유의한 차이가 없는 인자로는 남녀비, 임상증상, 입원기간, 치료성과, 결혼상태, 교육수준 및 직업등 이었다. 후기망상증군과 정신분열병군 사이에 통계적으로 유의한 차이가 있는 인자로는 초발연령, 입원당시 연령, 이병기간, 입원빈도, 사회적응도 및 자녀수등 이었다. 그러나 이러한 대부분의 차이는 정신분열병군의 초발연령이 후기망상증군보다 빠른 것과 밀접한 관련이 있을 것으로 생각된다. 따라서 정신분열병의 초반연령 제한만 없다면 후기망상증군은 정신분열병에 속할 수 있기 때문에 이들을 후기정신분열병등의 진단으로 정신분열병의 범주에 포함시키는 것이 더 적합한 것으로 생각된다.

Some clinical characteristics of 40 late-onset paranoid psychotic patients were compaired with those of schizophrenics. There were some similarities between the two groups in terms of sex ratio, clinical symptom characteristics, length of stay in hospital, response to treatment, marital status, education level and occupation. In contrast, age at onset and on admission, duration of illness, frequency of admission, adaptive functioning and numbers of children were different between two groups. These differences, however, may be explained by different age at the first breakdown of each psychotic group. These results suggest that the late-onset paranoid psychoses may be a subtype of schizophrenic disorder.
