인공판막심내막염의 임상적 고찰

Abstract

A retrospective examination was made of 13 patients with infected prosthetic cardiac valves of 320 Cardiac valve recipients at the Dongsan Medical center during 1980ㅡMarch 1988. Of these patients, 6 were male and & were female. The age of these patents were from 12 to 57 and the mean was 32 years old. Twelve Patients received cardiac valve at this medical center and the other one was transferred from other Hospital after valve replacement. Overall incidence of prosthetic valve endocarditis(PVE) was 3.7%. The incidence of PVE after mitral valve replacement was 3.7%, 6% after aortic valve replacement and 1.8% after double valve replacement. The Incidence of PVE in the recipients of tissue valves and mechanical valves were 3.9% and 1.6%, respectively. The causative organisms were founded s following ; pseudomonas species in 4 patients enterococcus species in two, Alcaligenus fecalis in two, Staphylococcus aureus in one, Aspergillus species in one, Cnadida rhizopos in one, and no growth in three. Overall mortality rate was 61.5%. But mortality rate in cases received antimicrobial therapy only was 75% and in cases with medico-surgical combined therapy was 40%.
