열공성 망막박리의 임상적 고찰

Abstract

Authors clincially analysed 50 patients(52 eyes) operated on for rhegmatogenous detachment of the retina at the Dong San Medical Center between May 1986 and August 1988. The results of study were as follows 1. Most of patients were between 10 and 39 years of age (62%) and men were affected more than women(2:1). 2. Involvement of the right eye was predominant(68%) and the incidence of bilaterality was 4%. 3. Myopia of -2D or more was found in 36.6% and of -6D or more in 21.2% and aphakia was seen in 11.5%. 4. Round holes were the most frequent(57.7%) and horse-shoe tears were 33%. One hole or tear was found in 57.7% of all cases. In site of breaks, round holes were mostly located in the temporal half of the retina and horse-shoe tears in the superior half and both types of breaks occured mostly near the equator. 5. The most frequent etiological factor was myopia and lattice degeneration(51.9%) and trauma was associated in 19.2%. 6. The overall success rate of reattachment was 88.5% and the main cause of failure was PVR. 7. A favorable surgical outcome was related to round hole in shape, short duration within 2 months, good pre-operative visual acuity and under 50 years of age. An unfavorable outcome was related to giant retinal dialysis, totally detached retina, ocular trauma and multiple operations. 8. The visual acuity was improved in 71.7% of cases of successful reattachment and the most frequent post-operative complication was PVR.
