군 신경정신과에서 의병전역한 환자들의 임상적 특징

- 요 약 저자들은 국군대구병원에서 1985년 9월부터 1989년 8월사이에 신경정신과 질환으로 의병전역한 환자 405명을 대상으로 그들의 임상적인 양상을 조사하여 다음과 같이 요약한다.
1) 진단별 분포는 정신분열병이 33.3%로 가장 많았고, 간질이 18.5%, 인격장애 17.3%, 주요정동장애 13.8%, 정신지체 10.4%의 순이었다.
2) 계급 및 복무기간별 분포에서는 이병이 59%, 입대 6개월이내가 64.2%로 군 복무기간중 초기에 의병전역하는 경향이 강했다.
3) 신경정신과적 치료의 과거력이 있는 경우에는 40.7%였고, 이들중 입대전의 경우가 전체 대상의 31.8%나 되었다. 또한 간질과 정신분열병은 입대전의 치료 과거력이 각각 46.7%, 37.8%로서 높은 빈도를 보였고, 특히 정신분열병의 17.8%는 입대전에 입원한 과거력을 가지고 있었다.
4) 문제행동의 과거력은 23.7%였고, 특히 인격장애와 주요정동장애에서 각각 51.4%, 50%로 높은 빈도를 보였다.
5) 발병시기에서는 입대전에 발병한 경우가 73.3%로 나타났다.
To understand and prevent the military service related neuropsychiatric causalities, the authors surveyed some clinical characteristics of 405 patients who were discharged from military service from Sep. 1985 to Aug. 1989 and who had been hospitalized at Neuropsychiatric Department of Taegu Armed Forces General Hospital prior to their discharge from the military service. The results were as follows; 1. The diagnostic distribution revealed that schizophrenia was most frequent as 33.3%, followed by seizure disoder 18.5%, personality disorder 17.3%, major affective disorder 13.8% and mental retardation 10.4%. 2. More than half of the patients(64.2%) were admitted to the hospital within six months of their military service enlistment. 3. The previous history of neuropsychiatric treatment prior to the enlistment in the army was found in 31.8% of all the subjects, 46.7% of the seizure disorders and 37.8% of the schizophrenics. Expecially, 17.8% of the schizophrenics had the history of neuropsychiatric hospitalization prior to the enlistment in the army. 4. The previous history of problematic behaviors was found in 23.7% of all the subjects, 51.4% of personality dis4roder, and 50% of major affective disroder. 5. The onset of illness prior to the enlistment in the army 73.3%. The results suggest that a thorough neuropsychiatric ebaluation of the enlistees may be to reduce and to prevent military service related neuropsychiatric causalities.
