- Abstract
- This study compared the clinical
efficacy as well as knee
ultrasonographic efficacy between nonsteroidal anti-inflammatory drug (NSAID)
and acetaminophen in the treatment of knee osteoarthritis. I administered 12 mg
of lornoxicam (plus 150 ㎍ of misoprostol) as NSAID or 1,950 mg of
acetaminophen in 45 randomly selected patients with knee osteoarthritis for 6
weeks. The effectiveness of these drugs on osteoarthritis was assessed by Western
Ontario and McMaster Universities Osteoarthritis Index (WOMAC score) and knee
ultrasonographic parameters (length of capsular distension and amount of joint
effusion) before and after 6-week treatment. Seven patients of lornoxicam group
and five patients of acetaminophen group were withdrawed and 33 patients
finished this study. The clinical symptoms of osteoarthritis assessed by WOMAC
score were improved by 6 weeks after treatment with both lornoxicam and
acetaminophen, but the lornoxicam group showed better clinical improvement than
acetaminophen group significantly (p<0.05). The length of capsular distension and
amount of joint effusion decreased after treatment in both groups, but, there were
no significant difference between lornoxicam group and acetaminophen group. The
findings showed superior efficacy of lornoxicam clinically. However there were no
differences in more objective parameters of efficacy (as ultrasonographic findings
of knee). Considering the safety and lower price of acetaminophen than NSAIDs, it is recommended acetaminophen as the initial therapy of osteoarthritis again.
