대뇌 피질 동맥 기원의 비 외상성경막하혈종

Abstract

요 약 84세 남자 환자가 투석 후 혼수상태를 주소로 본원 응급의료센터에 내원하였다. 조영 증강 뇌 전산화 단층촬영 혈관조영술에서 양측 뇌궁륭부의 경막하 출혈과 동맥 출혈이 관찰되었다. 경막하혈종은 주로 외상에 의해 발생한다. 그러나 외상의 병력이 없는 동맥 출혈에서 기원하는 경막하 혈종은 ‘급성 자발성 경막하혈종’으로 드물게 보고가 되고 있다. 비록 혈액 응고 장애, 동정맥기형 및 동맥류 파열도 비 외상성 경막하혈종의 원인이 될 수 있지만, 자발적 경막하 혈종의 경우에는 피질 동맥 기원의 가능성이 고려되어야 한다. 뇌 전산화 단층촬영 혈관조영술이 동맥 기원의 출혈 병소의 식별과, 다른 원인과의 감별진단에 도움이 될 수 있다.

An 83 year-old man was admitted to our hospital for management of a comatose mentality after hemodialysis. Computed tomographic angiography showed acute subdural hematoma (SDH) in both cerebral convexities with arterial bleeding. Subdural hematoma is usually caused by trauma. However, acute spontaneous subdural hematoma (ASSDH) of arterial origin without traumatic history is has been rarely reported. Although non-traumatic subdural hematoma may be caused by coagulopathy, arteriovenous malformation or ruptured aneurysm, the possibility of the cortical artery origin should be considered in case of spontaneous SDH. Computed tomographic angiography can help to identify the bleeding focus of the arterial origin, and to make a differential diagnosis from other origins.
