Angiotensin Receptor Blocker와 Calcium Channel Blocker의 복합 복용이 복강 내 이산화탄소 주입에 따른 혈압 변화에 미치는 영향

Abstract

Trendelenburg 자세로 복강경 수술을 받는 환자는 복강 내 CO2 주입에 따른 급격한 혈압 상승의 위험을 가지는데, 이는 주술기 합병증의 위험을 높일 수 있어 환자의 기저질환과 복용 약물 및 체액량 등에 대한 정확한 이해를 기반으로 수술 중 적절한 혈압 관리를 제공하는 것은 매우 중요하 다. 본 연구에서는 수술 전 항고혈압제의 복용이 수술 중 혈압 변화에 어떠 한 영향을 미치는지 알아보고자 하였다. 본원에서 복강경 하 대장 수술을 받은 환자들의 의료 기록을 후향적으로 수집한 결과 항고혈압제를 복용하지 않는 정상 환자군(N군), 고혈압 진단을 받고 calcium channel blocker(CCB) 만을 복용하는 환자군(C군), angiotensin receptor blocker(ARB)와 CCB를 복합 복용하는 환자군(AC군)으로 나눌 수 있었다. 이 중 저혈압 발생의 우 려로 인해 수술 전 지속 혹은 중단의 논란이 있는 ARB를 중심으로, ARB 와 CCB를 복합 복용하고 있는 환자군과 CCB 단독 복용군을 비교했고 또한 복합 복용군과 정상 환자군을 비교하였다. 본 연구에서 ARB와 CCB를 복합 복용하는 군은 정상 환자군과 비교했을 때 복강 내 CO2 주입 후 혈압 상승의 정도가 유의하게 작았고, 이때 환자의 나이가 혈압 변화에 유의한 영향을 미쳤으며 이를 공변량으로 처리하여 분석하였다. 또한 복합 복용군 과 CCB 단독 복용군을 비교했을 때도 복합 복용군에서 복강 내 CO2 주입 후 혈압 상승의 정도가 유의하게 작았다. 이는 ARB의 혈관 이완, 순환 혈 액량 감소, 교감신경 억제 작용이 복강경 수술 시 Trendelenburg 자세에서 의 복강 내 CO2 주입이 혈압에 관여하는 기전에 반대 작용하여 혈압 상승 을 약화시킨 것으로 보인다. 따라서 평소 ARB와 CCB를 복합 복용하는 고 혈압 환자가 Trendelenburg 자세에서 복강경 수술을 받는 경우, 수술 전 약제 복용을 수술 당일 아침까지 지속하는 것이 수술 중 복강 내 CO2 주입 에 따르는 혈압 관리에 긍정적인 영향을 미치는 것으로 생각된다

The patients who undergo laparoscopic surgery are at risk of a sudden increase in blood pressure due to intraperitoneal CO2 insufflation, which can increase the risk of perioperative complications, so it is very important to manage it properly. In this study, we investigated the effect of preoperative antihypertensive medication on the intraoperative blood pressure change. From the retrospective data, the patients who underwent laparoscopic colon surgery were divided into three groups: normal patients taking no antihypertensive, patients only taking calcium channel blocker (CCB), patients taking angiotensin receptor blocker (ARB) and CCB both. For there being controversy to continue or withhold ARB due to concerns about hypotension, we compared group AC with the other groups about the intraoperative blood pressure change. In this study, the blood pressure of the ARB and CCB combination therapy group rised significantly less after intraperitoneal CO2 insufflation compared to the normal patient group, and also the CCB alone group. It seems that the effect of ARB involving blood pressure offsets the effect of the intraperitoneal CO2 insufflation in the Trendelenburg position. Therefore, continuing ARB and CCB combination therapy until the day of surgery has positive effect on the blood pressure management under that situation.
