당뇨병과 연관된 심리사회적 문제들

Abstract

Psychosocial factors play an integral role in the management of diabetes in both children and adults. A number of psychosocial therapies can improve regimen adherence, glycemic control, psychosocial functioning, and quality of life. Many diabetic patients and their families have high rates of emotional disturbances and behavioral problems. Especially, hypoglycemia in children and chronic hyperglycemia in older diabetics cause significant neuropsychological impairment. Psychiatric interventions include pharmacotherapy, psychotherapy, psychoeducation, empowerment program, self-management training, stress management, and biofeedback etc. A multidisciplinary team approach, including physicians, mental health professionals, nurses, dieticians, pharmacists, and others, is needed for diabetics with some psychosocial issues.

당뇨병의 경과와 치료에 심리사회적 요인들이 매우 중요한 역할을 하며, 최근의 여러 연구들에서 많은 심리사회적 치료들은 처방 순응도, 당 조절, 심리사회적 기능, 삶의 질을 향상시킬 수 있음을 보여 주었다. 당뇨병 치료와 치료와 연관된 심리사회적 문제들은 성격, 대처방식, 정서적 상태(우울, 불안), 정신과적 장애(우울증, 식사장애, 불안장애), 사회적 지지, 스트레스 등이 포함된다. 많은 당뇨병 환자들과 그들의 가족에서 정서적 장애와 행동적 문제들의 발생률이 높고, 특히 아동에서는 저혈당이, 나이든 성인에서는 만성과혈당이 현저한 심경심리적 기능장애를 초래할 수 있다. 심리사회적 문제들을 가진 당뇨병 환자를 대할 때는 일상의 치료가 환자의 통상적인 생활, 환자와 가족의 행동, 환자와 가족들 간의 관계에 미치는 영향을 이해하며, 아동과 청소년 환자들에서는 당뇨병에 대한 적응문제를 주의깊게 살펴보아야 한다. 정신과적 개입으로는 약물치료, 개인정신치료, 집단치료, 가족치료, 정신교육, 능력부여 프로그램, 자기 관리 훈련, 스트레스 관리, biofeedback, 이완요법 등을 들 수 있다. 환자가 심리사회적 문제로 인해 의학적 치료에 지장을 받는다면 이런 환자들에 대해서는 내과의사, 간호사, 영양사, 정신보건 전문가 등이 서로 협력하는 통합적인 팀 접근이 필요할 것이다.
