DiaSTAT™을 이용한 Hb Aic 분석기의 평가 및 Micro-column test와 IMXR 을 이용한 Hb Aic와의 비교

Abstract

Background ： Measurement of Hb Alc has been recommended for the long-term assessment of glycemic control indiabetic patients. Because different analytical methods measure different glycated hemoglobin species, it has been difficultto compare results between laboratories. We compared the concordence of three different methods for determining Hb A,c and evaluated the DiaSTAT™ (Bio-Rad, U.S.A.) Hb A,c autoanalyzer, midget low pressure cation-exchange column chromatography. Methods ： The performance of DiaSTAT for measuring Hb Aic has been evaluated and compared with those of Micro-column test(Bio-Rad, U.S.A.)and IMxR Analyzer (Abbott, U.S.A.) based on boronate affinity binding. The results fromthree methods were compared statistically by linear regression analysis. Results : Within-run coefficients of variations(CVs) were 4.72% (at Hb Au level 5.4 %) and 3.84%(at Hb Alc level 9.9%), respectively. Between-run CVs were 4.82%(at Hb Alc level 5.4%) and 3.02% (at Hb Alc level 9.9%), respectively. The Hb Alc level from 55 healthy persons was measured. The 95% confidence interval ranged form 4.44

배경 : 당뇨환자의 장기간혈당조절 평가에 Hb A,e 측정이 유용한 방법으로다수의 측정법이 이 용되고 있다. Low pressure cation-exchange chromatography 법의 원리를 이용한 DiaSTAT™(Bio~ Rad, U.S.A.)기기의 평가를 위하여 boronate affinity ion capture assay를 이용한 IMxR(Abbott, U.S.A.)와 Micro-column test kit(Bio-Rad, U.S.A.)법 을 비교, 분석하였다. 방 법 : DiaSTAT의 정밀도와 참고치의 산정을 위하여 각각 Hb Alc농도가 5.4%와 9.9%인 대조검체를 이용하여 반복 측정하고, 건강 성인 55명을 대상으로• 참고치를 산정하고, DiaSTAT™, IMxR 및 Micro~column법 사이의 상관관계를 평가하고 회귀분석을 하였다. 결과 : DiaSTAT™로 측정된 두 가지 농도의 대조검체(5.4%, 9.9%)의 Hb A,c에서 검사 내 정밀도의 변이계수는 각기 4.72%, 3.84%로 측정 되었으며, 검사간 변이계수는 각기 4.82% 및 3.02%였다. DiaSTAT™기기를 이용하여 건강 성인을 대상으로 측정된 Hb Alc의 참고차(n=55)의 결과는 평균 5.313%였고, 95% 신뢰구간의 Hb Alc는 4.44%에서 6.19%였다. DiaSTAT와 IMxR의 상관관계는 y=0.661x+l.697(y:IMxR, x：DiaSTAT™, n=52) 이였고, 상관 계수 r=0.963(r2二0.9283)로서 매우 우수한 상관관계를 나타내었다. DiaSTAT과 Micro~column법 간의 상관관계는 y=0.735x+1.875 (y:Micro~column법，x : DiaSTAT, n=30)이였고, 상관계수 r=0.975 (r2=0.9502)로서 우수한 상관관계를 나타내었다. DiaSTAT에서 측정된 Hb Alc가 IMxR와 Micro~column법에서 측정된 값보다 각각 18.4%와 5.3% 높게 나왔으며, 측정치가 높을수록 차이값이 큰 경향을 보여 주었다. IMxR 및 Micro-column법의 상관관계는 y=1.001x-0.233(y:IMxR, x:Micro-column법, n=23) 이였고, r=0.839(r2=0.7046)의 상관관계를 나타내었다. 결론 : DiaSTAT에서 측정된 Hb Aᅵc값이 IMxR나 Micro-column법보다 다소 간 높게 측정되었으나, DiaSTAT와 IMxR, 그리고 DiaSTAT와 Micro-column법 간의 상관관계는 매우 우수하였다. 그러나 DiaSTAT™기종에서 측정된 Hb Alc값이 타 기종보다 Hb A,c 측정치에 비례하여 높게 측정 되었으므로 판독과 해석시에는 이를 고려해 야 할 것으로 사료된다
