구상돌기 골절에서 내측 접근법을 통한 전방 금속판 고정술 - 2예 보고 -

Abstract

Purpose: To report the clinical results of two cases of coronoid process fractures that were treated with volar plating through a medial approach. Materials and Methods: Two fractures of the coronoid process that needed to be fixed were managed with open reduction and internal fixation through a medial approach using 2.4 mm locking compression plates (Compact Hand set􀋓, Synthes, Switzerland). The patients were followed up for 14 months and 17 months and were evaluated using the Mayo Elbow Performance Score (MEPS). Results: The MEPS was 95 for Case 1 and 100 for Case 2. Active elbow joint motions were 5。-120。 (Case 1) and 0。-130。(Case 2). Supination and pronation fully recovered. Conclusion: Satisfactory results can be obtained in cases of coronoid process fractures because volar plating through a medial approach allows sound fixation and early mobilization of the elbow joint. 목적: 척골 구상돌기 골절에 대해 내측 접근법을 통한 전방 금속판 고정술이 시행된 2예의 임상 적 결과를 보고하고자 한다. 대상 및 방법: 수술적 치료가 필요한 척골 구상돌기 골절 2예에서 척 수근 굴근 양 두 사이로 접 근하는 내측 접근법을 통해 2.4 mm 잠김 압박 금속판 (Compact Hand set􀋓, Synthes, Switzerland)을 이용한 전방 금속판 고정술이 시행되었다. 추시 기간은 각각 14개월, 17개월이 었으며, Mayo Elbow Performance Score로 임상적 결과를 판정하였다. 결과: Mayo Elbow Performance Score는 각각 95점 (증례 1), 100점 (증례 2)이었다. 능동 적 주관절 운동 범위는 5􀆆~120􀆆, 0􀆆~130􀆆이었고, 회내 및 회외 영역은 모두 회복되었다. 결론: 내측 접근법을 통한 전방 금속판 고정술은 척골 구상돌기 골절에서 견고한 고정 및 조기 관 절운동을 가능하게 하여 우수한 치료 결과를 얻을 수 있었다. 색인 단어: 구상돌기, 골절, 2.4 mm 잠김 압박 금속판,전방 금속판 고정술
