Harris-Galante 2형 비구컵을 이용한 무시멘트성 인공고관절 전치환술

Abstract

Purpose : To assess the follow up results of more than five years of total hip arthroplasty using the Harris-Galante K acetabular cup.

Materials and Methods : We performed a clinical and radiographic assessment of 126 cases of total hip arthroplasty done with the second-generation Harris-Galante cup between 1990 and 1996.

Results : The average follow up period was 7.1 years. The average Harris hip score improved from 49.5 points preoperatively to 89.7 points at the latest follow up. Radiographic analysis revealed excellent osteointegration into the cup in 123 hips(97.6%), while 3 cups were unstable. Periacetabular osteolysis was noticed in 19 cases(15.1%). Radiolucent lines developed in 11 cases(8.7%). Fifteen cups(11.9%) were revised. The average linear polyethylene wear rate was 0.23 mm/year.

Conclusion : The midterm results of using the second-generation Harris-Galante acetabular cup shows excellent clinical and radiological results although further follow up is required due to complications such as significant wear of the liner as well as occurrence of osteolysis.

목 적 ： Harris-Galante II형 비구컵을 이용한 일차적 고관절 전치환술을 시행하고 5년 이상 추시가 가 능하였던 환자들을 대상으로 그 추시 결과를 알아보려 하였다.

대상 및 방법 ： 1990년 6월부터 1996년 9월까지 Harris-Gatante II형을 사용하여 인공고관절전치환 술을 시행하고 5년 이상 추시 관찰이 가능하였던 104명 126례를 대상으로 Harris 고관절 점수를 측정 하고 방사선 투과선, 비구컵의 안정도, 골용해 및 폴리에틸렌 라이너의 마모 정도를 관찰하였다. 결과: 평균 7.1년 추시시 Harris 고관절 점수는 술전 평균 49.5점에서 최증 추시시 89.7점으로 향상 되었으며 최종 추시시 비구컵의 고정도는 97.6%에서 골성 고정 소견을 보였고 3례(2.4%)에서 불안정 성을 보였다. 비구컵 주위의 골용해는 19례(15. 1%) 에서 관찰되?;U 2 mm 이하의 방사선 투파선은 11 례(8.7%)에서 보였다. 비구컵의 재치환술은 15례(11. 9%) 에서 시행되었다. 라이너의 면평균 선상 마모 율은 0. 23 mm/year 이었다.

결 론 ： HarrisᅳGalante M형 비구컵은 7년 추시상 골내 성장은 잘 유도되나, 폴리에틸렌 마모율의 증 가와 비구컵 주위의 골용해가 많고 이로 인한 재치환 빈도가 높으므로 주의 깊은 추시 관찰이 요구된다.
