대퇴골두 무혈성괴사에 시행한 양극성 반치환술과 고관절 전치환술 - 후향적 비교연구 -

Abstract

Purpose: A matched pair study was undertaken, retrospectively, to compare the results of treatment between a bipolar hemiarthroplasty and a total hip arthroplasty for osteonecrosis of the femoral head. Materials and Methods: Thirty matched pairs were evaluated with respect to the follow up period, sex, age, weight, bone type and activity level. There were no differences in the immediate postoperative fixation status for the stem or the size of the implant. The clinical and radiographic results, after a mean follow up of 10 years, were compared. Results: There were no differences in the Harris hip scores and the incidences of thigh pain. The radiographic analysis revealed no difference in the fixation status at the latest follow up, but higher incidences of femoral osteolysis, revision rates and acetabular complications with bipolar hemiarthroplasty. Conclusion: The use of bipolar hemiarthroplasty is not recommended due to the high incidences of osteolysis, revision rates and acetabular complications in the treatment of osteonecrosis of the femoral head.

목적: 대퇴골두 무혈성괴사시 무시멘트성 양극성 반치환술과 고관절 전치환술의 치료 결과의 차이를 비교하기 위해 match 방식을 통한 후향적 분석을 시도하였다. 대상 및 방법: 추시 기간, 성별, 연령, 몸무게, 골형태, 활동성 등의 기준에 따라 반치환술 군 및 전치환 술 군 각각 30예가 match 되었으며 사용되어진 스템의 크기, 수술 직후 고정도의 차이는 없었다. 평균 10년 추시상 임상적 및 방사선 결과를 비교하였다. 결과: Harris 고관절 점수 및 대퇴부 동통은 차이가 없었다. 방사선학적으로 최종 추시시의 대퇴 스템 고정도는 두 군간에 차이가 없었으나 대퇴골용해는 반치환술시 46.7%, 전치환술시 23.3%에서 나타났으며 재치환술의 빈도는 반치환술시 40%, 전치환술시 16.7%였고 비구측 합병증의 빈도도 반치환술에서 높게 나타났다. 결론: 대퇴골두 무혈성괴사의 양극성 반치환술은 골용해, 비구측 합병증, 재치환술의 빈도가 높으므로 그 사용에 신중을 기하여야 할 것으로 사료된다.
