최소 침습 술식을 이용한 일차성 고관절 전치환술 (예비보고)

Abstract

Purpose: To evaluate the effectiveness of a minimally invasive (MIS) primary total hip arthroplasty. Materials and Methods: Primary total hip arthroplasties were performed in 86 patients between January 2002 and June 2002. The incision length, the positions of the femoral stem and acetabular cup, operative time, transfusion units, postoperative rehabilitation time and complications were compared between the MIS (19 cases) and traditional incision (19 cases) groups. Results: The average incision lengths were 7.1 and 16.7cm in the MIS and traditional incision groups, respectively. The postoperative positions of the femoral stem and acetabular cup were satisfactory in all cases. The average operative time and transfusion unit were 90 minutes and 0.7 units and 106.3 minutes and 1.8 units in the MIS and traditional incision groups, respectively. No intraoperative or postoperative complications were observed in any of the cases. Conclusion: Minimally invasive surgery, in total hip arthroplasty, has the advantages of smaller incisions, smaller amounts of blood loss and shorter rehabilitation times. Further follow up is required to assess the long term complications.

목적: 최소 침습 술식(minimally invasive surgery, MIS)을 이용하여 인공 고관절 전치환술을 시행하고 그 효용성을 분석해보고자 하였다. 대상 및 방법: 2002년 1월부터 2002년 6월까지 일차성 인공 고관절 전치환술을 시행한 86예의 환자 중최소 침습 술식(MIS)을 이용하였던 19예의 환자군과 전통적인 절개선을 이용하였던 19예의 환자를 대조군으로 하여 수술 절개선의 길이, 방사선학적 대퇴 스템 및 비구컵의 위치, 수술에 소요되는 시간, 수혈양, 술후 재활에 소요되는 기간, 술후 합병증 등을 관찰하였다. 결과: 절개선의 길이는 최소 침습 술식 군에서 평균 7.1 cm, 대조군에서 평균 16.7 cm이었으며, 수술 후 시행한 방사선학적 검사에서 대퇴 스템 및 비구컵은 두 군 모두에서 만족할만한 위치를 보였다. 최소 침습 술식 군에서 수술에 소요된 시간은 평균 90분이었고 대조군에서는 평균 106.3분이었으며, 수혈양은 최소 침습 술식 군에서 평균 0.7 unit, 대조군에서 평균 1.8 unit였다. 술후 재활에 소요되는 기간도 최소 침습 술식 군에서 대조군 보다 짧았다. 술중 및 술후 합병증은 전예에서 추시 기간 중 발견할 수 없었다. 결론: 인공 고관절 전치환술에서 최소 침습 술식의 이용은 수술 절개선의 길이 및 수혈양을 줄일 수 있으며 술후 재활에 소요되는 기간을 단축시켜 주는 등의 장점이 있는 것으로 나타났으나 장기간의 추시 관찰을 통해 발생 가능한 합병증에 대한 평가가 이루어져야 할 것으로 사료된다.
