견관절 탈구와 동반된 동측 상완골 간부 골절 - 증례 보고 -

Abstract

Dislocation of the shoulder with ipsilateral humeral shaft fracture is very rare, but serious injury that requires emergent care. There have been approximately 20 cases reported in the English literature, but it has never been reported in Korea. We report a case of dislocation of right shoulder with ipsilateral humeral shaft fracture which was successfully treated by closed reduction of the shoulder under general anesthesia and internal fixation with antegrade interlocking intramedullary nailing for the humeral shaft fracture. 견관절 탈구와 동반된 동측 상완골 간부 골절은 매우 드물게 발생하나 응급 처치를 요하는 심한 외상 중의 하나이다. 현재까지 전세계적 으로 약 20예의 증례가 보고되어 있으나 발생 기전과 치료 방법에 있어 명확하게 정해진 것이 없으며, 국내에서는 아직 보고된 증례가 없다. 이에 저자들은 우측 견관절 전방 탈구와 동반된 동측 상완골 간부 골절로 내원한 환자 1예를 경험하였고 전신 마취하에 견관절의 도수 정복을 시행한 후 상완골 간부 골절에 대해 도수 정복술 및 전향성 교합성 골수강내 금속정 고정술을 시행하여 좋은 결과를 얻었기 에 문헌 고찰과 함께 보고하고자 한다. 색인 단어: 견관절 탈구, 동측 상완골 간부 골절
