미만성 식도경련의 임상 및 식도내압검사소견

Abstract

Objectives: The present study was performed to evaluate the clinical and monometric findings of diffuse esophageal spasm.

Methods : The clinical and manometric findings of 17 patients with diffuse esophageal spasm who diagnosed by esophageal manometry with solid state catheter were studied from August 1988 to August 1994 in gastrointestinal motility laboratory of Dongsan Hospital.

Results : The incidence of diffuse esophageal spasm was 3.2%(ll/537cases). Male to female ratio was 1:3.3. Common chief compliants were dyspha-gia(8cases), chest pain(7cases), and globus sense (2cases). Most of the patients responded to drug therapy except one case who undertaken pneumatic dilatation. Associated manometric findings were repetitive contraction^ cases), hypertensive lower esophageal sphincter(4 cases), high amplitude contraction^ cases), contraction of long durationd case), retrograde contraction 1 case). Amplitude of simultaneous contractions was significantly lower than peristaltic contractions (35.0±18.4mmHg vs 103.8±40.7mmHg).

Conclusion: Diffuse esophageal spasm was an uncommon esophageal motility disorder with dysphagia and chest pain and was associated with nonspecific manometric findings.

목 적 ： 미만성 식도경련은 최근까지 진단기준이 모 호했던 일차성 식도 운동질환으로, 동반되는 식도내압 소견이 다양한 것으로 알려져 있다. 이에 저자들은 미 만성 식도 경련환자의 임상양상 및 식도내압소견을 알 아보기 위해 이 연구를 시행하였다.

방 법 ： 1988년 8월부터 1994년 8월까지 계명대학 교 동산병원 소화관운동 검사실에서 식도내압검사로 진단된 025환자 17예에 대해 임상양상 및 식도내압 소견을 조사하였다.

결 과 ： DES환자는 같은 기간에 식도관련중상으로 식도내압검사를 시행한 환자중 3.2%를 차지하였고 남 녀비는 1:3.3이었으며 증상은 연하곤란이 8예(47.1%)로 가장 많았다. 약물치료로 대부분 증상의 호전이 있 었으며 1예에서는 풍선확장술을 시행하였다. 동반된 식 도내압소견은 반복성 수축파 6예, 고압성하부식도 괄약 근 4예, 고진폭연동수축파 2예, 장기간 연동 수축파， 역행성 수축파가 각각 1예씩 관찰되었으며 동시수축파 의 진폭은 연동수축파에 비해 유의하게 낮게(35.0±18.4mmHg vs 103.8±40.7mmHg)나타났다.

결 론 ： DES는 여자에 많고 연하곤란, 흉통올 호소 하며 다양한 식도내압 소견을 동반하는, 흔하지 않은 식도운동질환이다.
