양성 및 악성 식도협착환자에서 내시경적 식도풍선 확장술의 치료효과

Abstract

Objective: This study was undertaken to evaluate the therapeutic effect of endoscopic balloon dilatation in patients with benign and malignant esophageal strictuires.

Methods: 125 endoscopic dilatations with Rigiflex ® TTS Balloon Dilator were performed in 47 patients with benign and malignant esophageal strictures in Keimyung University Dongsan Hospital from January 1992 to December 1993. 33 patients with benign strictures included caustic damage (16/33), postoperative (14/33), esophageal ring (1/33), and reflux esophagitis (1/33). 14 patients with mlignant strictures included intraluminal esophageal cancer (12/14) and external compressive cancer of lung and breast (2/14).

Results: The results of endoscopic balloon dilatation in 33 patients with benign esophageal strictures were "Good" in 21 cases, "Fair" in 9 cases and "Poor" in 3 cases. The results of 14 patients with malignant esophageal strictures were "Good" in 8 cases, "Fair" in 3 cases and "Poor" in 3 cases. The symptomatic improvement of benign and malignant strictures was shown in 90.9% and 78.6%, respectively.

Conclusion: Endoscopic balloon dilatation is a safe, simple, and effective method for primary treatment of benign and malignant esophageal strictures.

목적:양성 및 악성 식도협착환자에서 내시경적 풍 선확장술의 치료효과를 관정하고자 했다.

방법 ： 1992년 1월부터 1993년 12월까지 계명대학교 동산의료원 내과에서 양성 식도협착환자 33예와 악성 식도협착환자 14예로 진단된 47명을 대상으로 Rigiflex® TTS (Through The Scope)Dilator로 총 125회의 풍선확장술을 시행하였다. 양성 식도협착환자는 양잿물에 의한 부식성 협착이 16예, 수술후 문합부 협착이 14예, 식도 web이 1예, 식도 ring이 1예 이었다. 악성 식도협착환자는 식도암이 12예, 외부 압박성 종양이 2예인데 폐암이 1예, 유방암 이 1예이었다.

결과:양성 식도협착환자 33예는 “Good” 21예， “Fair“ 8예,"Poor" 4예로 87.9%의 중상호전율을 보 였다. 악성 식도협착환자 14예는 “Good” 8예， "Fair" 3예, "Poor” 3예로 78.6%의 증상호전율을 보 였다.

결론:양성 및 악성 식도협착환자에서 내시경적 풍 선확장술은 안전하고, 간편하며，효과적이며 일차적으 로 시행할 수 있는 치료방법이라고 생각된다.
