일차성 식도이완불능증 환자에서 공기풍선확장술 전후 식도내압검사소견의 변화

Abstract

Objectives : The present study was undertaken to compare esophageal manometric findings before and after pneumatic dilatation and to determine whether manometric changes can be useful for evaluation of therapeutic response.

Methods .' Sixteen patients with primary achalasia were examined esophageal manometry with solid state catheter before and 2 months after pneumatic dilatation using the Rigiflex achalasia balloon dilator 35 mm.

Results : Lower esophageal sphincter pressure was decreased from 45.5±16.2 mmHg to 19.7±6.2 mmHg and the gradient between intraesophageal pressure and gastric baseline pressure was reversed from 11.8±7.2 mmHg to -2.6±3.9 mmHg (p<0.001). Lower esophageal sphincter relaxation and partial return of esophageal peristalsis were observed in 50% and 31.2% of cases respectively after dilatation, compared to 12.5% and 0% before dilatation.

Conclusion : Esophageal manometric parameters changed significantly, which could be usefull for evaluation of therapeutic response after pneumatic dilatation in patients with primary achalasia. However, longterm follow up study should be done in larger number of patients. Key Words: Achalasia, Manometry, Dilatation

목 적: 일차성 식도이완불능증의 진단에 필수적인 식도내압검사 소견의 각 지표가 공기풍선확장술 치료 2개월후 어떻게 변화되며，술후 치료효과의 판정에 유 용한 지를 알기 위해서이다.

방 법: 이전에 근절개술이나 공기풍선확장술을 받은 적이 없는 일차성 식도이완불능중 환자 16예를 대 상으로 solid state catheter를 이용한 식도내압검사를 실시하고， Microvasive사 Rigiflex Achalasia Balloon Dilator 35mm를 사용하여 공기풍선확장술을 시 술하였다. 시술 2개월후 추적검사를 실시하여 식도내 압검사 지표의 변화를 조사하였다.

결 과: 하부식도괄약근압은 술전 45.5土16.2 mmHg에서 술후 19.7±6.2mmHg로 감소하였고 (p<0.001), 식도체부내압과 위기저압의 차이는 술전 11.8±7.2mmHg에서 술후 -2.6土3.9mmHg로 역 전되었다(p<0.001).

하부식도괄약근의 이완은 술전 12.5%의 환자에서 있었으나, 술후 50%의 환자에서 나타났고, 연동파의 부분회복은 술후 31.2%의 환자에서 나타났다.

결 론:일차성 식도이완불능중 환자에서 공기풍선 확장술후 식도내압검사 지표의 변화는 하부식도괄약근 압의 감소와 식도체부내압과 위기저압의 차이의 역전 등이 유의했으며，이는 술후 치료효과의 판정에 유용할 것으로 생각된다, 하부식도괄약근의 이완과 연동파의 부분회복 역시 일부에서 나타났다. 따라서 앞으로 더 많은 예에서 최소한 1년 이상의 장기추적검사가 필 요하다고 생각된다.
