황열 백신 접종 후 발생한 뇌수막염 1예

Abstract

Yellow fever is the original viral hemorrhagic fever (VHF), a pansystemic viral sepsis with viremia, fever, prostration, hepatic, renal, and myocardial injury, hemorrhage, shock, and high lethality. Yellow fever was one of the most feared lethal diseases before the development of an effective vaccine. Yellow fever (YF) can be prevented by an attenuated vaccine. The yellow-fever 17D vaccine developed in the 1930s has been regarded as one of the most successful live attenuated vaccines, with few side effects or adverse events. The adverse effects associated with yellow-fever vaccine are generally mild and include headache, myalgia, and low-grade fever. Recently, however, some cases of severe neurologic disease and multi-organ system disease have been described in individuals who received yellow-fever vaccine. We report the case of a 39-year-old female with meningitis following vaccination with 17D yellow-fever vaccine. (Korean J Med 76:S204-S207, 2009) Key Words: Yellow fever; Headache; Meningitis

황열(Yellow fever)은 황열바이러스감염에 의한 급성 열성 질환으로 주로 아프리카나 중 남미의 열대 우림지역에서 유 행하며 대부분이 경한 증상을 보이나 10~20%에서 신부전, 간부전, 현저한 서맥을 동반한 고열이 나타나는 전형적인 황 열 증상을 나타낸다. 황열은 치명적인 감염 질환으로 황열 백신이 개발되기 전까지는 20~50% 정도의 환자가 사망하였 다. 황열 백신은 상당히 안전한 백신으로 접종 부위의 통증 과 부종, 미열, 근육통 등의 경미한 부작용 외 중증의 부작용 이 드문 것으로 알려져 왔으나 중증의 합병증이 간염, 뇌염, 다장기 부전, 길리안 바레 증후군 등의 형태로 외국문헌에 보고된 바 있으나 아직까지 국내에 보고된 바는 없다. 이에 저자는 국내에서 처음으로 황열 백신 접종 후 뇌수막염이 발생한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이 다. 드물지만 황열 백신 접종 후 두통이나 발열 발생 시 뇌 수막염이 발생할 수 있으므로 반드시 감별질환에 포함되어 야 하겠다. 중심 단어: 황열; 두통; 뇌수막염
