3형 와덴버그증후군

Abstract

Purpose: We report a case of Type 3 Waardenburg syndrome accompanied by intermittent exotropia, which had not previously been encountered in Korea along with the literature study Methods: A 5-year-old boy visited as a result of hypochromic heterochromic iridum in the right eye. Suspecting Type 3 Waardenburg syndrome, plain radiography, a hearing test, an alternate prism cover test and fundus examinations were performed and the presence of lateral displacement of the lacrimal puntum was evaluated. Results: The presence of hypochromic heterochromic iridum, dysplasia of the left thumb, sensorineural deafness, dystopia canthorum along with a lateral displacement of the lacrimal puntum, albinism of the posterior pole upon a fundus examination, and intermittent exotropia with a 20prism diopters of deviation were found. Conclusions: This patient showed the 4 criteria for a diagnosis of Type 3 Waardenburg syndrome including hypochromic heterochromic iridum, dystopia canthorum along with lateral displacement of the lacrimal puntum, sensorineural deafness, and a muscular system abnormality, i.e., dysplasia of the left thumb. He also showed intermittent exotropia.

목적 : 국내에서 아직 보고 된 바 없는 간헐외사시를 동반한 3형 와덴버그증후군(Waardenburg syndrome)을 경험하 였기에 이를 문헌고찰과 함께 보고하는 바이다. 대상과 방법 : 5세 남아가 우안의 저색소성 홍채이상을 주소로 내원하여 3형 와덴버그증후군 의심 에 X선 촬영, 청력 검사, 내안각간 거리와 누점의 외측편위 여부, 프리즘교대가림검사 및 안저검사를 실시하였다. 결과 : 저색소성 홍채이색증, 좌측엄지의 이형성증, 감각신경 난청, 내안각간 거리 증가와 누점의 외측편위, 안저검사 상 우안 후극부의 백색증, 20 프리즘디옵터의 편위각을 가진 간헐외사시 소견을 보였다. 결론 : 본 증례는 저색소성 홍채 이색증, 내안각이소증과 누점의 외측편위, 감각신경성 난청, 좌측 엄지의 이형성 같은 근육계 이상의 3형 와덴버그증후군의 주 진단 기준 4가지를 보였고 간헐외사시도 동반하였다.
