엑시머 레이저를 이용한 근시교정술의 효과

Abstract

To evaluate the effect of excimer laser photorefractive keratectomy on the correction of the myopia and the corneal endothelium, 158 eyes which underwent myopic photorefractive keratectomy with excimer laser (Excimed UV 200, Summit Technology Inc., Waltham, USA were analyzed. Myopic patients were divided into 4 groups according to their refractive errors and operative techniques (group I : -2.00D~-6.00D, group I :-6.25D~-9.00D, group I : >=-9.25D, group IV ： >=-6.25D and 2 times of irradiation). Refractive error, visual acuity, keratometry, pachometry, 10P, corneal haze and morphology of the corneal endothelium by specular microscopy were examined during the follow up period. Twelve months after surgery, mean uncorrected visual acuities of the group I， 1, I and F (9months after surgery) were 0. 9, 0. 7, 0.2 and 0. 7 respectively. Mean reduction of refractive errors were 4. ID, 6.3D, 7. ID and 9.2D respectively. Correction within ± 1.0D were achieved in 93.3%, 60%, 0% and 66.1% respectively. Mean reduction of keratometric diopters were 4.0D, 4.8D, 5.5D and 4.8D and mean reduction of corneal thickness were 37.2µm, 50.4µm, 57.0µm and 67.2µm respectively. Corneal opacity remained to insignificant degree below grade 0.3. There was no statistically significant change in morphometric changes of the corneal endothelial cells.

Excirner Laser에 의한 근시교정술의 효과 및 각막내피세포에 미치는 영향울 알아보기 위하여 중심 부 각막절제술을 시행한 126명 158안올 대상으로하여 근시정도와 수술방법에 따라 4군으로 분류하고 〔1군:-2.00D~-6.00D，2군:-6.25D~-9.00D, 3군: >=-9.25D, 4군:>=-6.25D(2 step)〕, 수술전 후의 시력, 굴절교정의 정도, 각막혼탁，각막굴절력，중심각막두께, 안압 및 각막내피세포의 형태학적 변화를 관찰하였다. 술후 12개월의 평균 나안시력은 1군에서 0.9, 2군에서 0.7，3군에서 0.2, 4군(술 후 9개월)에서는 0. 7이었고，평균 굶절력은 각각 4.1D, 6.3D, 7.1D, 9.2D 감소하였다. 각군의 각 막굴절력의 변화는 4.0D, 4.8D, 5.5D, 4.8D 감소하였고，중심각막두께는 각각 37.2µm, 50.4µm, 57.0µm, 67.2µm 감소하였다. 각막혼탁은 4군을 제의한 모든 군에서 Grade 0.3이하로 감소하였고 각 막내피세포의 수술 전후의 형태학적 특성은 유의한 변화를 보이지 않았다.
