양측 서혜부 성선을 동반한 남성호르몬 불감 증후군( 완전형 ) 1례

Abstract

Androgen insensitivity syndrome is a genetic syndrome characterized by complete or partial resistance of end organ to the peripheral effect of androgen. Patient have a male karyotype(46,XY) and bilateral testes. Appearance of external genitalia depend on the degree of androgen insensitivity. In the complete form, external genitalia is normal female but in the incomplete form, external genitalia figure varies hom that of a virilized female to that of an undervirilized male with a short penis and hypospadia. The gonads are able to be located along the descending course of the testis during development. The usual presenting symptom in complete type is primary amenorrhea or inguinal mass, but in incomplete type, the patient mainly present with ambiguous genitalia. Recently we experienced one case of this syndmme and removed the bilateral gonads, so we report it with brief review of literatures. Key word : Androgen Insensitivity Syndrome

남성호르몬 불감증후군은 남성호르몬의 작용에 대한 말초 조직의 완전 또는 불완전 저항성에 의해 야기 되는 질환이다. 환자들은 정상 남성 핵형 46, XY와 양측성 고환조직을 가지고 외부 생식기의 형태는 남성호르몬에 대한 저항의 정도에 따라 달라지며 완전형에서는 정상 여성의 형태를 나타내나 불완전형에서는 남성화된 여성의 형태에서 미약한 남성화를 보이는 남성의 형태 즉 왜소음경 및 요도하열 등의 다양한 외부 생식기 소견을 보인다. 성선의 위치는 고환의 하강 궤적을 따라 어디에도 존재할 수 있으며 일반적으로 자각 증상은 완전형에서는 일차성 무월경과 서혜부 종물 불완전형에서는 주로 모호한 외부 성기의 형태로 나타난다. 최근 저자들은 양측 서혜부 성선을 동반한 남성호르몬 불감증후군(완전형)으로 진단된 일차성 무월경 환자 1례를 경험하여 양측 성선 절제술을 시행하고 문헌 고찰과 함께 보고하져 한다.
