식도운동질환과 위식도역류질환에서 산청소능 비교 -저진폭수축파 비특이성 식도운동질환을 중심으로-

Abstract

Background/Aims: The aim of this study was to evaluate acid clearance in esophageal disease, a common cause of noncardiac chest pain, and to evaluate the correlation between the swa]lowing number for acid clearance and the amplitude of contractions. Methods: The study population was divided into four groups. Group 1(n=8) was controls with no complaints of any gastrointestinal symptoms. Group II(n=9) was gastroesophagea1 reflux disease. Group III(n=6) was nutcracker esophagus. Group IV(n=7) was nonspecific esophageal motility disorder with low amp]itude contractions. We performed standard acid clearance test and compared it with the amplitude of esophageal contractions. Results: In Group I, the swal]owing number was 6.12+ 0.58(mean+ SE); in Group II, 15.33 3.96; and in Group lll and IV, 9.83 + 2.70 and 20.42 ' 4.43 respectively. The number of swallowing was significantly increased in Group IV than in the other groups(P=0.05) and showed the reversed correlation with amplitude(r=-0.81, p=0.026). Conclusions: Group IV had significantly delayed esophageal acid clearance and also showed the reversed correlation with amplitude.

목적: 비심장성 흉통의 주요 원인으로 대두되는 식도질환에서 식도 산청소능을 조사하고 아울러 식도수축파 진폭과의 상관관계를 알기 위해 본 연구를 시행하였다. 대상 및 방법: 대상환자들은 4군으로 분류하였으며, 제 I군은 정산대조군 8예, 제 II군은 위식도역류 환자군 9예, 제 III군은 호두까기 식도증 환자군 6예, 제 IV군은 식도내압 소견상 저진폭 연동 수축파 소견을 보이는 비특이성 식도운동질환 7예를 대상으로 하여 표준산청소능검사를 시행하고 식도수축파 진폭과 비교하였다. 결과; 각 군간의 연하 횟수는 정상대조군에서 6.12±0.58회(mean±SE), 위식도역류 질환에서 15.33±3.96회, 호두까기 식도증에서는 9.83±2.70회 였으며, 저진폭수축파 소견을 보이는 비특이성 식도운동질환에서는 20.42±4.43회로, 저진폭수축파 소견을 보이는 비특이성 식도운동질환에서 정상군에 비해 유의하게 증가된(p<0.05) 소견을 보였으며, 식도산청소 검사의 연하 회수와 진폭사이에 유의한 역상관관계(상관계수: -0.81, p=0.026)를 보였다. 결론: 저진폭연동수축파소견을 보이는 비특이성 식도운동질환에서는 정상군에 비해 산청소능이 지연되었으며, 식도산청소 검사의 연하회수와 진폭과의 관계도 유의한 역상관관계를 보였다.
